  Quality Planning and Quality Improvement
  Activities
• Facilities rank lowest in Staff and Consumer
  Involvement and Quality Program Evaluation
• It is difficult to draw meaningful conclusions
  from the trends analysis due to a small number of
  third-year follow-up assessments
• There seems to be general improvement among the
  facilities that had second-year follow-up
  assessments conducted
• If the small number of third year assessments are
  indicative of any real trend, then sustainability
  is an issue, especially regarding performance
  measurement
