3Choosing indicators for impact monitoring
- Impact indicators are selected in accordance
with the projects goals and objectives using a
conceptual ME framework, depending on the type
of malarial situation. Impact indicators are
epidemiological indicators which reflect the
trends in infection propagation. They include - Death rates associated with malaria all-cause
under-5 mortality rate in highly endemic areas - Incidence of clinical malaria cases (estimated
and/or reported) - Prevalence of malaria-transmitting parasites

- Number of clinical (symptomatic) cases of malaria
number of people diagnosed with uncomplicated
or not severe malaria, who are receiving
treatment for malaria in accordance with the
national malaria treatment principles - Justification
- In persons with insufficiently strong immune
systems, malaria infection typically leads to
clinical illness of which acute fever is the most
common symptom. Malarias disease burden is best
reflected in the type of the disease and not
prevalence. - Measurement
- The total number of cases of malaria registered
in the national health information systems. - Sources The national medical data gathering
system. - Frequency Monthly in absolute figures and
intensive indicators and annually with an
accompanying analysis of the state of sentinel
surveillance
5Impact indicator Number of pockets of malaria
- Indicator definition
- Number of active pockets of grade 2 vivax malaria
- Number of active pockets of grade 2 falciparum
malaria - Justification
- A pocket of malaria (not a case of malaria) is
considered to be the basic unit to which measures
can be applied. The effectiveness of such
measures can be assessed via pocket category
dynamics. Therefore, monitoring of malaria, the
entomological situation and the impact of
anti-malarial measures, is carried out by
examining pockets. - Measurement
- The total number of active pockets of grade 2
malaria identified by the countrys
parasitological service during epidemiological
studies of pockets, sentinel surveillance data on
the malarial situation in the pocket over the
last 3 years. - Sources The national medical data gathering
system. - Frequency Routine reporting and epidemiological
map by case and pocket category on case detection
and annually with an accompanying analysis of the
state of sentinel surveillance. -

7Solving ME problems that arise during project
implementation
- Overall, monitoring and evaluation of project
implementation involved the use of indicators
that are part of the existing national malaria
epidemiological surveillance system, and used by
the GF structure, therefore no significant
problems were encountered in the area of ME. - Before the project began such coverage indicators
as the number of households treated with
insecticides and the number of people protected
with domestic treatment were not used but details
of such work were recorded on the ground. -
- In order to deal with this issue, departmental
reporting form 16, which included 2 additional
columns for the above indicators, was prepared
and submitted to the Ministry of Health.
