1Filling Out Entry Tags
2Club Name Your club name needs to be at the top
of all entry tags.
3Lot Lot numbers can be found in the State Fair
Book.
4Project Name This is the name of the project area
you are exhibiting. The name is listed right
beside the Lot in the 4-H State Fair book.
Examples are Visual Arts, Foods and Nutrition,
or Electricity.
5Member ID Your ID number is on your 4-H
membership card. Your membership card was in the
packet your family received from the Extension
Office and was attached to the Achievement Day
Handbook.
6County Name This information will help to
identify your exhibit. Your county name is
Minnehaha.
7Member Name Please print your first and last name.
8Personal Information Please enter your date of
birth in the first section. Then please put your
4-H age in the appropriate blank. Your 4-H age
is the age you were as of January 1st, 2006.
Next, check the appropriate blank for if you are
a Beginner, a Junior, or a Senior. Beginners8-10
years old Juniors11-13 years old Seniors14-19
years old
9Additional Information Enter the number of years
in the Project and Lot. You may also add any
detailed description of the exhibit.

11Ribbon Placing Section You do not need to
complete this section. It will be completed on
Judging Day.
12Back of Tag This is where you can explain your
process for completing your entry. Judges do
check for information to assist in your final
placing.
Filling out the back of the tag can make a
difference in a ribbon placing! When completing
the back of your exhibit card, put something
between the cardboard sheet and the blue paper.
This will prevent your writing from marking on
the front of the card.
13What did you do? Explain the process for
completing your exhibit. What tools did you use,
what steps did you take, etc.
14What did you learn? Did you use a new tool? Did
you try a new technique? Did you learn a new
skill? Did you have a mess-up that you learned
from, such as dont mix the muffins too much or
they become tough.
15What would you like the judge to know? Share any
information that may help the judge in reviewing
your project, such as how you completed the
exhibit and your intensions for use of the
exhibit.
16Additional Information Only certain exhibits need
to complete this section so check the 4-H State
Fair book to determine if you need to complete
this part.
