    that disadvantaged communities and groups are

What you have said so far
  • In 2008 it was agreed that the VCS focus should
    be on obesity, older people and carers
  • Important to remember that Healthy Glos Strategy
    is not the only health related work in the county
  • VCS have a contribution to make at all levels
    from strategic to hands-on delivery

Joint Commissioning
  • Joint Strategic Needs Assessment (JSNA)
  • Ensuring that services are shaped by local
    communities, inequalities are reduced and social
    inclusion is increased
  • World Class Commissioning better outcomes for
    people
  • VCS feeds into this commissioning process via
    representation at the GHCWP
  • JSNA currently looking at Obesity, Older People,
    Learning Disabilities and Children Young People

Reducing Health Inequalities
  • Comprehensive Area Assessment review of the
    GHCWP
  • GHCWP aims to narrow the gap in health outcomes
    between communities and groups living in our
    disadvantaged and more affluent areas
  • Glos Local Area Agreement (LAA) focuses on
    reducing health inequalities through lifestyle
    factors, and supporting independence for older
    and vulnerable people
  • Strategic leadership role of GHCWP needs to be
    reviewed
  • Links between district and county not always
    clear
  • Clear targets for health inequalities need to be
    set

Putting People First Transforming Social Care
  • Demography living longer but with more complex
    conditions and chronic illnesses
  • Higher expectations greater control
  • Changing family structures changes to caring
    responsibilities
  • Finances need to explore options for longterm
    funding of care

What does personalisation mean?
  • Choice and control over the shape of support
  • Cannot be done with social care in isolation but
    needs to be jointly done across housing,
    benefits, leisure, transport, health.
  • Needs partnership with a range of providers
    including the VCS
  • Implications for the VCS - organisations will
    need to redesign local services around needs of
    communities

  • Shaping the Future of Care Together - first ever
    national debate on the reform of adult care and
    support in England
  • Spells out a vision for a National Care Service,
    the options for  reform, and how the new system
    could be organised and paid for.
  • More joined up working, wider range of care and
    support and better quality/innovation
  • Various funding proposals
