8 A feltáplálási szindróma
- Eloször 1944-ben, a burmai japán fogolytáborok
amerikai felszabadítása után brit hadifoglyokon
észlelték. - Ancel Keys 1944-46 között 6 hónapig limitált
diétán tartott 36 önkéntes éhezon szerzett
kísérletes (az ú. n. Minnesota Starvation
Experiment) tapasztalatait írta meg évekkel
késobb kétkötetes könyvében. (Az éhezést
3-hónapos rehababilitációs periodus követte.) A
tapasztalatai alapján rekonstruálták, hogy mi is
történhetett a fogolytáborokban. - /Keys, A.The Biology of Human Starvation.
(Vol. 1. 2., 1950)/ - Éhezéskor (malnutritioban) a szövetek csökkent N
tartalma korrelál az IC ionok
koncentrációjával. Helytelen feltáplálás során
(refeeding syndrome) súlyos, extracellulárisan
is manifesztálódó elektrolit hiány
(hypophosphataemia, hypokalaemia,
hypomagnesaemia) alakul ki a malnutritios
betegeken, ami gyakran fatális kimenetelu. - Magyarázat nagy mennyiségu N bevitele
következtében az IC ionok részarányosan a
sejtekbe vándoroltak - Megoldás a fehérje bevitel csökkentése
és/vagy a IC ionok bevitelének növelése.

átlagos évi súlyvesztesége 4 volt. - A dementia csökkent táplálék felvételhez vezet

- Barrett-Connor és mtsai 20 éves követéssel 199
pácienst vizsgáltak, akik közül 60 esetben
mérsékelt-közepes fokú dementia alakult ki
(valószínusítheto volt az Alzheimer-kór).
Igazolható volt, hogy akiknél (késobb) dementia
alakult ki elotte szignifikáns testsúly vesztés
lépett fel, míg a kognitív szinten intaktaknál ez
nem következett be! A fogyás nem volt kapcsolatba
hozható sem az életvezetéssel, sem depresszióval,
sem más betegségekkel. Vagyis a testsúly
csökkenés megelozte a dementiát. A malnutritio a

- TBK-dependent models. Although Moore's model is
the first suggested BCM estimation approach and
is still widely applied, there remain fundamental
questions related to this classic model. First,
Moore's model assumes a stable K/N ratio of 3
mmol/g. However, individual tissues and organs
vary widely in their K/N ratios, from a low of
0.45 mmol/g in skin to a high of 5 mmol/g in the
brain (22). The corresponding whole body K/N
ratio is 2.0 mmol/g (i.e., 3,580 mmol/1,800 g)
in Reference Man (17). . - Therefore, the measurement errors of the two
TBK-dependent models (0.64 kg for the TBK-TBW
model and 0.59 kg for the simplified TBK model)
are comparable to the error of the improved Cohn
BCM evaluation method (0.63 kg). - Accordingly, the present study does not support
a significant difference in the measured TBK/BCM
ratio between healthy adult men and women (109.4
6.9 vs. 108.9 11.3 mmol/kg, P gt 0.05). - However, the simplified TBK model may include
considerable model error. The suggested constancy
of the TBK/BCM ratio (109.1 mmol/kg) is based on
assumptions that all four determinants are
stable, a 0.70, KICW 152 mmol/kg, KECW
4 mmol/kg, and E/I 0.95. Although these
assumptions are necessary for deriving the
TBK-only prediction method, any variations in the
four determinants across subjects may cause a
corresponding model error for the simplified TBK
method. Future validation studies are needed to
evaluate the relationship between TBK and BCM in
populations outside of healthy adults, including
children and patients with underlying diseases. - Conclusion. On the basis of the cellular-level
body composition model, a BCM prediction equation
was derived from TBK and TBW measurements. The
model-derived magnitude of the TBK/BCM ratio was
109.1 mmol/kg, identical to the value measured in
healthy subjects with an improved version of
Cohn's BCM prediction model (109.0 10.9
mmol/kg). A simplified model was thus suggested
as BCM (kg) 0.0092 x TBK (mmol). -

- This theory proposes that aging is due to
changes in neural and endocrine functions that
are crucial for 1) coordinating communication and
responsiveness of all body systems with the
external environment 2) programming
physiological responses to environmental stimuli
and 3) maintaining an optimal functional state
for reproduction and survival while responding to
environmental demands. These changes, often
detrimental in nature, not only selectively
affect the neurons and hormones that regulate
evolutionarily significant functions such as
reproduction, growth, and development, but also
affect those that regulate survival through
adaptation to stress. Thus the life span, as one
of the cyclic body functions regulated by
"biological clocks," would undergo a continuum of
sequential stages driven by nervous and endocrine
signals. Alterations of the biological clock,
e.g., decreased responsiveness to the stimuli
driving the clock or excessive or insufficient
coordination of responses, would disrupt the
clock and the corresponding adjustments (27, 28,

- An important component of this theory is the
perception of the hypothalamo-pituitary-adrenal
(HPA) axis as the master regulator, the
"pacemaker" that signals the onset and
termination of each life stage. One of the major
functions of the HPA axis is to muster the
physiological adjustments necessary for
preservation and maintenance of the internal
"homeostasis" (steady state) despite the
continuing changes in the environment (7, 12).
During life span, chronic exposure to severe
stress from a multitude of physical, biological,
or emotional stimuli may exhaust or weaken the
capacity to adapt and lead to the so-called
"diseases of adaptation" and death (58, 82).
Aging would then result from "a decreasing
ability to survive stress," one of the many
definitions of aging that suggests a close
relationship between stress and longevity.

- With aging, a reduction in sympathetic
responsiveness is characterized by 1) a decreased
number of catecholamine receptors in peripheral
target tissues 2) a decline of heat shock
proteins that increase stress resistance in many
animal species, including humans, and 3) a
decreased capability of catecholamines to induce
these heat shock proteins (93). The hormones of
the adrenal cortex are glucocorticoids, for the
regulation of lipid, protein, and carbohydrate
metabolism mineralocorticoids, for that of water
and electrolytes and sex hormones. Among the
latter is dehydroepiandrosterone, which decreases
with aging dehydroepiandrosterone replacement
therapy has been advocated in humans, despite
unconvincing results (19). Glucocorticoids, as
well as other (ovarian and testicular) steroid
hormones, are regulated by positive and negative
feedback between the target hormones and their
central control by the pituitary and
hypothalamus. With aging and in response to
continuing and severe stress, not only feedback
mechanisms may be impaired, but also
glucocorticoids themselves may become toxic to
neural cells, thus disrupting feedback control

- The Neuroendocrine Theory has recently been
supported by data showing that an "ancestral"
insulin pathway controls stress responses and
longevity in the nematode C. elegans (39).
Mutations of a number of genes in this pathway
confer 1) resistance to environmental stress,
including heat shock (93), 2) enhanced resistance
to starvation, and 3) extended longevity. Many of
these same genes are conserved in humans the
insulin/insulin-like growth factor-I (IGF-I)
peptide and Daf-2 gene are homologs of the human
insulin and IGF-I receptor, unc-64 and unc-31 are
homologous to human synthaxine and catabolite
activator proteins that are involved in the

Age-1 is related to a conserved
phosphoinositol-3-kinase that responds to insulin
receptor activation, and Daf-16 is the homolog of
the human forkhead box, class-O transcription
factor (5). In C. elegans, a relatively complex
organism, these genes constitute a primordial
neuroendocrine system in which the insulin/IGF-I
peptide integrates information from environmental
stress. The resulting integrated responses play
an important role in monitoring metabolic and
reproductive status to permit appropriate energy
adjustments and, ultimately, extend life span
(87). Thus it may be assumed that this primitive
neuroendocrine system has the capacity not only
to coordinate what occurs in each cell and tissue
of the body, but also to avoid disorganization
(e.g., overexpression leading to toxicity) of
stress responses. These landmark studies in
nematodes encourage further exploration of
hierarchical programming among the multiple

- We can speculate as to whether this may
contribute to the weight loss. There is also
evidence that patients with DAT have a general
loss of homeostatic regulatory mechanisms, such
as thermoregulation and cardiovascular reflexes,
which can reduce the ability to conserve energy
(101). A 6-y longitudinal study correlated weight
loss with the degree and progress of DAT and with
mortality, whereas weight gain was related to a

- One RCT showed the results of diet therapy for
dementia patients with PEM at a British
psychiatric hospital (98). Of nearly 300
patients, 80 (27) were underweight. Of these, 46
were included in a randomized treatment study
using liquid dietary supplements of 600 kcal/d (
2500 kJ/d) for 12 wk. The intervention group
gained an average of 3.5 kg, whereas the control
group maintained a stable weight. No evaluation
of functional capacity or mortality was
performed. The controversial use of tube feeding
in cases of severe dementia was recently reviewed
(111). A retrospective study of individuals with
severe cognitive disorders in nursing homes found
no survival advantages in those who were tube-fed
compared with those who were not (112). In
conclusion, the nutritional treatment of PEM
associated with dementia has been insufficiently

- Nutrition plays a major role in the development
and maintenance of bone structures resistant to
usual mechanical loadings. - In addition to calcium in the presence of an
adequate vitamin D supply, proteins represent a
key nutrient for bone health, and thereby in the
prevention of osteoporosis. - Experimentally selective deficiency in dietary
proteins causes marked deterioration in bone
mass, micro-architecture and strength, the
hallmark of the osteoporosis disease. - Clinically large prospective epidemiologic
studies indicate that relatively high protein
intake is associated with increased bone mineral
mass and reduced incidence of osteoporotic
fracture. - Low protein intake is often observed in
patients with hip fracture and intervention study
demonstrates that following orthopedic
management, protein supplementation attenuates
post-fracture bone loss, increases muscles
strength, reduces medical complications and
hospital stay. - There is no consistent evidence for superiority

- Eight studies of postoperative nutritional
treatment of hip-fracture patients are summarized
in Table 5 (120127). All studies were
controlled and randomized, but the randomization
procedure was not always clearly described. Two
studies described the treatment of established
PEM in hip-fracture patients (125, 126), whereas
the other studies did not use PEM as an inclusion
criterion. Four dietary supplement studies
reported a shorter hospitalization period for
those who received supplements (120, 123, 124,
126). Two studies from the same researchers
showed fewer postoperative complications, such as
infections (123, 124). One study indicated
beneficial effects on mortality (121). However,
the number of deaths was small. In one of the
studies, researchers noted that a 4-wk treatment
with anabolic steroids (nandrolone) had no effect
(127). A meta-analysis of 943 patients aged gt65 y
performed by the Cochrane Library (128) concluded
that there was evidence of positive effects of
oral protein and energy supplements. In general,
however, the quality of the studies was assessed
as low and further studies were requested. In
conclusion, oral or enteral postoperative
treatment with balanced or protein-rich
nutritional supplements (protein supplement of
20 g/d) for 34 wk may shorten hospitalization
periods in hip-fracture patients. /Akner G,
