  • Diagnoses and treats acute/chronic disorders
  • Makes referrals to other specialists
  • Must be available 24 hours a day to provide

Who is Your Primary Care Provider (PCP)?
  • Under the Health Plan you must pick a Primary
    Care Provider (PCP).
  • This is the person who keeps all your records and
knows the most about your health.
  • If you dont pick a PCP your health plan will
    pick one for you.
