government, public and private donations,
contracts, grants, payments from patients and
insurance companies. Sliding fee discounts to
those who qualify.   Services offered include
Dental, Internal Medicine, Family Planning,
Family Practice, Laboratory Testing, Mammograms,
Mental Health/Substance Abuse Treatment, OB/GYN,
Ophthalmology, Pediatrics, On-Site Pharmacy,
Podiatry, STD Testing Treatment, Translation,
Transportation, Patient education, Social
Services, Case Management, Medicaid
Eligibility.   Population Served Primarily
low-income African Americans, Hispanics,
Caucasians. Single parents, school-aged
children, elderly, and the chronically ill.

AIM By implementing the components of the care
model, we will redesign our clinics practice so
that at least (1) At least 70 of our patients
with diabetes are involved in self-management
activities
                     
(2) Have at least two HbA1c levels below 7.0 and
(3) At least 40 of our diabetic patients will
have BP readings of less than 130/80.

POPULATION OF FOCUS            The initial
Population of Focus will be all of the active
Diabetic patients of Dr. Matewere (Provider
Champion).         This includes approximately
160 patients with diabetes.         Additional
patients will be included in the POF as they are
identified.

supported the team? How has middle management

Directors?   Leadership has provided resources
(IS, staffing, time, education, funds,
flexibility to work on collaborative activities
and positive reinforcement).             Middle
management has coordinated all pre-work meetings,
reminded team of responsibilities, pre-work chart
data entry.      The Centers CEO reminded
the Board of Directors over and over and
over and over again of the Centers
participation, progress and benefits to the
patient and to the Center for its
participation in the collaborative.

COMMUNICATION The Leadership Team sent out a
series of email messages over the Centers LAN
lines, through the sophisticated technology of
handheld PC interfaces, syncd to the internal
email software, which was downloaded each day as
the staff members and Board members either turned
on the computer or used his/her telephone and
