What happened FA-EE BRUISES KNEE AS WELL HEAD
CAGE TURNS OVER ON TRAILER. A crew was installing
cages around well heads in order to protect them
from upcoming completions work. A trailer with
two well cages was delivered to location. The
first cage was successfully offloaded and the
second pre-slung cage was ready for crane hook
up. As the rigger and assisting truck driver
were standing on the trailer they heard a loud
pop and someone called out. Both employees
jumped from the bed of the truck. As rigger
jumped for safety, he scraped his knee. The cage
then fell onto its side. At that time both
employees had cleared the area.

• The trailer should have never been used due to
  the deteriorated condition of the planking.
• Hazard Recognition was not utilized to Stop the
  Job.
• Contractor asset integrity inspections on
  equipment is poor.
• Driver failed to follow DOT regulations.
