Terms of Reference
  • Provide strategic leadership to EoE patient
    safety agenda
  • Support and facilitate a culture where patient
    safety is a key priority for providers and
    commissioners of care
  • Ensure good clinical engagement in the other
    programme boards, embedding a safety culture and
    creating a sense of urgency for top patient
    safety priorities
  • Develop/adapt patient safety guidance and lead on
    patient safety projects as agreed by Steering
    Group
  • Align the Board with local and national campaigns
    with agreement on metrics
  • Promote patient safety awareness and culture at
    executive and Board level in providers and
    commissioners to demonstrate leadership in
    patient safety
  • Work with patient safety champions

Patient Safety CPB Priority Areas (1)
  • Based on implementation plan and Terms of
    Reference deliverables
  • Sustain improvement and reduction in
    healthcare-associated infections extend from
    MRSA and C difficile. Include surgical site
    infections and ventilator-associated pneumonia
  • Improve VTE prevention, reduce hospital venous
    thromboembolism and improve anticoagulant control
    across health systems set up project group and
    completed first 90 day challenge
  • Improvement in the management and outcomes of
    acutely ill and deteriorating patients, including
    in Mental Health project group set up, two
    meetings

  • A reduction in adverse drug events and errors,
    together with improvements in medicines
    reconciliation and general medicine management.
    Consideration of an East of England wide acute
    drug chart. Project group being set up through
    east of England pharmacy networks
  • A reduction in overall hospital mortality within
    the region is an overall aim of the programme
  • Further priorities being developed over time
  • Including ensuring the development of support
    systems, sharing of good practice via the
    development of a Faculty.

    Clinical Programme Board and build awareness
