  • Arose because of a manpower crisis
  • Recruiting and training cohorts (in first year)
  • Defined structure to balance training and
    apprenticeship
  • Very intensive training and assessment

  • No competitive hurdle between SHO and SpR
  • Robust check at the end of year 1 to quality
    assure progression
  • End of Year (Stage) Review

ARCP has replaced ESR
  • The paperwork
  • ESSR documents progress against relevant
    standards the form is good and user-friendly

    , teaching etc)

  • All trainees can get 18 satisfactory WBA
  • Poor trainees may get an acceptable MSF
  • Passing the OSPE is not enough to progress
  • Almost all trainees carry out an audit an do
    teaching

  • The professional view of trainers is important!
  • The Composite Trainers Report is very important
    and should explicitly be part of the process

  • Workload is also important
  • The College has produced targets for workload
    (500 surgicals, 20 post mortems, 300 cytology)
  • Had they been applied rigorously last year many
    would not have progressed no flag at ARCP

ARCP to see or not to see?
  • The Histopathology Training Schools Board
    unanimously agreed that Year 1 Histopathology
    Trainees should be seen face-to-face at ARCP
  • WHY?

Why see trainees?
  • No stigma in having to attend
  • 8-9 months into a new specialty
  • Reinforce achievements during the year (good or
    bad)
  • If paperwork is incomplete / incorrect can
    resolve simply and quickly
  • Guarantee to sign off paperwork same day

Why see trainees?
  • We look after trainees in year 1
  • They do everything else together
  • Agree remedial training plans if required and
    could facilitate cross-boundary support
  • RCPath does not trust trainers view to sit the PM
    exam (and surgicals?)
  • The professional views of trainers are important
    and should be recognised

Why not see trainees?
  • Because it says so in the guidance
  • Because its quicker (if everything goes to plan)
  • Because reports written by educational
    supervisors are always fully accurate and
    completeand they always know what they are doing

  • Large centre with panel of examiners including
    lay and external representative
  • Standardised review with every trainee
  • Most did not have ESSR form fully completed
  • At least one trainees paperwork raised eyebrows

  • ESSR completed for every trainee
  • Detailed training plans agreed for every trainee

  • What are the issues?
  • Basically a good process supported by good
    paperwork
  • Do all educational supervisors know what is
    required of them?

  • Current documentation could be improved (e.g.
    explicit documentation of workload, professional
    view of trainers) in the context of the
    curriculum

  • What are the issues?
  • Just because the experts say we cant see
    trainees does not mean that they are right
  • and they may be after year 1 anyway

  • Sufficient documentary evidence should be
    available to make a full judgement of a trainees
    competency
  • That should be made easier by actually having the
    trainee in the room
  • Trainees dont all look the same
