When will the survey be given?

• Students in the 16 Franklin County school
  districts and 41 non-publics, grades 8th, 10th
  and 12th, will take the survey between November
  30 and December 16.
• Our school will administer the survey on

Materials for Teachers

• Classroom Teacher Instructions
• Teacher Script
• Post Survey Classroom Checklist
• Envelope with red printing and Envelope with blue
  printing(for returning materials to Building
  Coordinator)

Review Classroom Teacher Instructions

• Before survey - review instructions
• During survey- monitor classroom conditions and
  record survey conditions on Post Survey Classroom
  Checklist, noting any highly unusual conditions
  that occurred
• After survey- return Checklist and Envelopes to
  Building Coordinator

Using the Two Envelopes

• Envelope with red printing
• Complete the lower portion, place all
  distributed surveys complete and incomplete in
  envelope and seal it.
• Envelope with blue printing
• Place undistributed surveys in this envelope
  and the completed Post Survey Classroom
  Checklist.

Immediately return both envelopes to your
Building Coordinator.

Day of Survey

• Make sure that everyone has a 2 pencil.
• Stress that circles should be filled and erasures
  complete.
• Read Teacher Script.
• Hand out surveys
• Collect surveys and return to Building
  Coordinator

Key Points for Classroom Teachers

• Administer during designated class period,
  allowing adequate time (35 minutes).
• Do NOT discuss the questions with the students
  before, during or after survey.
• Stress that the survey is ANONYMOUS - NO NAMES.
  We want to know OVER ALL about students, not
  about any ONE student.

Key Points (continued)

• Do not walk around the room while students
• are filling out survey.
• Collect surveys in such a way to maintain
  confidentiality (e.g.responsible student collects
  them)
• This is NOT a test answers arent right or
  wrong.
• Dont answer questions about specific substances.

Key Points (continued)

• Students hand in surveys even if they didnt
  answer all questions.
• Students MAY NOT KEEP the survey
• No one in Ohio will see the surveys.
• Keep disruptions and interruptions to a minimum
  until everyone is finished
