Background
  • The program was authorized under the Public
    Health Service Act, Title II, Section 330A(g) in
    2002
  • ORHP delayed the start of this program to
    incorporate recommendations from the rural IOM
    report, Quality Through Collaboration The
    Future of Rural Health, released in November
    2004.
  • The two-year program began in fiscal year 2006
    15 awards were made (up to 50,000/year).
  • The program was re-competed in fiscal year 2008
    55 new awards have been made (up to
    75,000/year).

Purpose
  • The purpose of the Rural Quality Grant Program is
    to improve patient care and chronic disease
    outcomes for diabetes mellitus (DM) and
    cardiovascular disease (CVD) by assisting rural
    primary care providers with the implementation of
    quality improvement strategies.
  • The program focuses on DM in year one and CVD in
    year two.
  • Improvements are achieved by
  • Use of an electronic patient registry system to
    track and report health indicators
  • Conducting organizational needs assessments to
    identify areas of improvement and implementing
    additional quality improvement activities
  • Participating in monthly technical assistance
    calls and 4 peer-learning workshops with fellow
    grantees
