  • I have designed a Dominos Order Form for the
Auburn University Website. Students will have to
log in to place orders. After a student has
logged in he/she will be able to place an order
at Dominos Pizza. Once he/she has placed an
order they will be shown their order and asked
which form of payment will they like to use and
if it will be carry-out or delivery. The website
will also contain coupons for the customers to
print out and use as well as a store locator so
customers will know which Dominos is located
    nearest them.

  • New Accounts- If a customer does not have an
account they must register so that Dominos can
keep a account of their transactions.
  • Existing Accounts- A customer with an account
just logs on and proceeds to placing an order.
  • Forgot Passwords- If a customer forgets his/her
password Dominos will email them their password.
  • Personal Info- In order for Dominos to put a
customer in the database the customer must fill
out the personal info form. This form consists
of surname, first name, address, city, zip
code, email address, telephone, sex student id,
visa , MasterCard , and expiration date. This
form will help Dominos speed up the process of
you placing your order. Once a customer is
placed in the Database, their previous orders are
stored as well as their credit card information
and address. This was done in a attempt to make
placing an order on-line quicker than calling on
    the telephone.
  • Pizza- Once you are ready to order you can

you would like a specialty pizza. Various
Specialty Pizzas are Feast Pizza, Veggie Pizza,

as to what form of payment they will be using as
well as if it will be carry-out or delivery.
  • Add to Shopping Cart- Once a customer makes a
selection on the order they can click the add to
shopping cart button so that their items will be
    placed in the shopping cart.
  • Check Out- Once a customer is finished shopping,
all of the items they placed in the shopping cart
will be displayed so they can confirm their order
    and it can be processed.
  • Quantity- This enables the customer to select
more than one of any particular item.
  • Form of Payment- Dominos accepts Tiger Card,
Visa Card, Master Card, Cash, and Checks.
  • Coupons- Coupons have been placed on the website

  • This is the initial page of the Dominos Order
Online Form. If you are making a new account you
will be asked to input your zip code. If you
already have an account you will enter your user
name and password. If you have forgotten your
user name and password it will be emailed to you.
You can also just look at the menu, coupons, or
store locator if you are not sure what you want
to order, or where the closest Dominos is
    located.

6
This is for a new member to input their personal
info so that Dominos can quickly process their
orders online.
7
As you can see the second part of the personal
info form prompts the user for additional
information in regards to the location of their
residence.
8
The Menu page displays all of the items Dominos
has to offer.
9
This is the order form for the customer. It
gives the option to select different pizzas,
sides, drinks, and salads.
10
This is a continuation of the order form. It
enables the customer to add the items they select
to a shopping cart or check out.
11
This is the shopping cart which allows the
customer to view their orders before they check
out.
12
At the bottom of the shopping cart page the
customer can select which form of payment and if
the order will be carry-out or delivery.
