Title: Dynamic Splinting

1
Dynamic Splinting
2
Dynamic splinting principles
  • high profile vs. low profile
  • what type of pulley to use

3
(No Transcript)
4
  • what type of outrigger to use
  • wire, splinting material, Phoenix
  • make outriggers round for strength
  • apply solvent prior to adhering outrigger
  • heat outrigger in splint bath and splint base
    with heat gun

5
  • which joints are immobilized and which are to be
    moved
  • dorsal hand tissue is thin
  • Splint should never put more that 50g/cm2
  • for leather sling with skin contact of 4 cm2, the
maximum pull should be 200 g.
  • smaller sling can tolerate less pull

6
Line of Pull
  • Pull should be at 90 degrees
  • Pull should follow natural movement of joint
  • Blocks might need to be incorporated into the
splint to provide the right amount of pull.
  • If splint is made to increase ROM, splint must be
monitored to maintain 90 degree rotational force.
  • Use eye screw, screwed into plastic outrigger to
    direct line of pull

7
What provides the traction
  • Rubber band
  • sizes
  • Springs
  • Elastic string
  • Static pull on fishing line attached to Velcro
  • glue fabric hooks or Velcro to finger tips
  • Rubber band anchored to splint with fabric hook
    or splinting material hook

8
Low load prolonged stretch
  • Research done by Kenneth Flowers and others
  • LLPS
  • Focus on total end range time (TERT)
  • Stay in dynamic or static progressive splint 8-12
    hours per day.

9
JAS
  • Joint Active System
  • Bonutti did research on this design
  • Wear splint ½ hour TID
  • High force used

10
Types of dynamic splints
  • flexor tendon splint
  • extensor tendon splint
  • MP flexion assist splint
  • anti-claw splint
  • radial nerve splint

11
