3Birth to Three Referrals
- Disabilities Services Division is the central
point of referral for birth to 3 in Milwaukee
County. - Referrals are assigned to service providers for
screening, evaluation and /or follow up.

- The earlier a childs developmental delays are
discovered, the more opportunities for the child
to increase their skills and abilities.

- Parent/guardians must give written consent.
- Participation is voluntary.

- Children age birth to 3 years of age, who
- Have a 25 delay in 1 or more areas of
development behavioral, social, cognitive,
speech/language and motor - Have a diagnosed condition with a high
probability of resulting in a developmental delay - Have a diagnosed hearing impairment or vision
impairment

14What happens after the evaluation?
- If the child is eligible for services an
Individual family Service Plan (IFSP) is
developed with the family. - This plan outlines the familys concerns
regarding the childs development and what
strategies and early intervention services will
be provided to address these concerns. - The service coordinator assists the family in
locating the recommended services. -

- Services are based on each childs Individual
Family Service Plan (IFSP). - The services are designed to help the family
with their childs developmental needs and to
provide learning opportunities for the child as
part of their daily routine.

- Staff have specialties in education, psychology,
occupational therapy, physical therapy, speech
therapy, social work and parent education. - Parent support groups and parent education
groups are also available.

- The Birth to 3 objective is to provide service
in a natural environment for the child and family.

- Other community agencies
- If a person other that the parent/guardian makes
a referral discus with parent prior to making the
referral.
