Health and Developmental ReviewsEvidence
suggests key transition points that provide
logical opportunities to review CYPs health.
  • The core purpose of health and development
    reviews is to
  • assess the childs, young persons and
    familys strengths, needs and risks
  • give children, YP, parents carers
    opportunity to discuss concerns and aspirations
  • assess physical health, growth development
    immunisation status
  • assess any mental or emotional health issues
    and
  • ensure appropriate support is available to
    meet educational /health needs such as language
    communication.
  • It is recommended that these reviews occur at the
    following transition points
  • reception/Year 1 (age 45) school entry
    assessment (transition from 05 HCP school entry
    questionnaire
  • Year 6/7 (age 1012) assessment at
    transition from primary to secondary school
  • mid-teens, when YP are embarking on the next
    transition stage.
  • In addition, reviews are described for
  • LAC
  • children YP with disabilities/special
    educational needs (SEN) at transition.

  • Lead role for the Health Visitor, working across
    Childrens Centres and General Practice in
    Pregnancy to First Five years of Life
  • 5-19 yr section delivered in schools by School
    Health Teams key link between health, education
    and wider CYPs services in most cases will
    have school nurses at core
  • Multi-skilled team working involving a range of
    practitioners across services
  • Up to date knowledge and skills
  • Working in partnership with children and families
  • Working effectively across service boundaries
