13Health and Developmental ReviewsEvidence
suggests key transition points that provide
logical opportunities to review CYPs health.
- The core purpose of health and development
reviews is to - assess the childs, young persons and
familys strengths, needs and risks - give children, YP, parents carers
opportunity to discuss concerns and aspirations - assess physical health, growth development
immunisation status - assess any mental or emotional health issues
and - ensure appropriate support is available to
meet educational /health needs such as language
communication. - It is recommended that these reviews occur at the
following transition points - reception/Year 1 (age 45) school entry
assessment (transition from 05 HCP school entry
questionnaire - Year 6/7 (age 1012) assessment at
transition from primary to secondary school - mid-teens, when YP are embarking on the next
transition stage. - In addition, reviews are described for
- LAC
- children YP with disabilities/special
educational needs (SEN) at transition.

- Lead role for the Health Visitor, working across
Childrens Centres and General Practice in
Pregnancy to First Five years of Life - 5-19 yr section delivered in schools by School
Health Teams key link between health, education
and wider CYPs services in most cases will
have school nurses at core - Multi-skilled team working involving a range of
practitioners across services - Up to date knowledge and skills
- Working in partnership with children and families
- Working effectively across service boundaries
