GOAL 1OUTREACH
  • Enrollment via engagement of Veterans in their
    community or surrounding areas.
  • Registration assist with the completion of form
    1010 for healthcare.
  • Primary Care and Mental Health service
    connection.
  • Provide additional information and assistance on
    form 21-526 (with the support of VSO offices).
  • Assist in the navigation of the Department of
    Veterans Affairs systemor at least refer to the
    appropriate department.
  • Reach out to as many Veterans in Rural American
    as is possible.

GOAL 2EDUCATIONAL ENHANCEMENT
  • Increase the veterans knowledge of, compliance
    with and awareness of chronic diseases such as
    Cardiovascular Disease (CVD), Congestive Heart
    Failure (CHF)and Diabetes Mellitus (DM).
  • Present and provide education and guidance on
    healthcare information that is geared towards the
    9 healthy choices as defined by VHA National
    Center for Health and Disease Prevention.

    and NCA services.
