AIMS

• To prevent Adverse Drug Events (ADEs)
• by implementing medication reconciliation at
  admission in Burns Lake Hospital
• Goals
• To increase the effectiveness of the medication
  admission reconciliation process over time
• To reduce the rate of Unintentional and
  Undocumented Intentional discrepancies at
  admission by 75 in 6 months
