Case
  • 66 yo Male with Hx of refractory ITP after
    steroids, IVIg and splenectomy.
  • 2005 CT scan followed by PET which notes an FDG
    avid sub-pectoral lymph node
  • Excisional bx demonstrates Marginal zone
    lymphoma.
  • ITP spontaneously resolves
  • 2009, acute recurrence of thrombocytopenic
    purpura. Plt count 9. PET negative.
  • Tx with Plt transfusion and Rituxan x 1 dose.
    Rapid resolution of thrombocytopenia, c/b macular
    rash on day 3 covering chest and trunk (no
    mucosal).

  • Thrombopoiesis is regulated by cMPL adsorbtion of
    TPO.
  • TPO drives increased ploidy Megs.
  • Megs abut bone marrow blood vessels.
  • Megs podocytes extend into the vessel and plts
    are released by sheer forces.
  • MPV gt 11, PDW gt 15 are highly specific for a plt
    destruction process (eg ITP).
