Vision
  • Decrease the prevalence of diabetes by educating,
    motivating and empowering individuals to make
    behavioral and lifestyle changes.

Mission
  • Improve the health of the underserved and
    uninsured of Oklahoma County by preventing or
    delaying the onset of Diabetes in those at risk.

Prevalence
  • 20.8 million people in the US have diabetes
  • Hispanics are 1.7 times more likely to have
    diabetes than non-Hispanic Whites
  • Approximately 190,000 Hispanics in Oklahoma
  • 16,000 (9) diagnosed with diabetes in 2006

  • Approximately 55,000 Hispanics in Oklahoma County

DPOP Classes
  • Classes are based on the Diabetes Prevention
    Program (DPP) Core Curriculum
  • Each class starts with the review of activity and
    food diaries
  • How Am I Doing Graphs
  • One hour class on selected topic
  • Clients are invited to stay after class to
    exercise

Developing Program
  • 2 rolling 6 week periods within a 12 week program
    (minimum attendance)
  • Clients are encouraged to complete 12 weeks

Class Goals
  • Client Outcome Goals
  • Reduce weight by 5
  • Engage in 2 ½ hours of activity a week
  • Program Goals
  • 50 of clients have 5-7 weight loss
  • 50 will lower fasting glucose by 3
  • 90 reduction in Tobacco use
  • 30 w/ cholesterol 200-250 will reduce below 200
  • 50 w/ cholesterol 250-300 will reduce below 225
  • 50 w/ cholesterol gt300 will reduce below 250
  • 50 w/ BP gt140/90 will reduce by 10 mmHg

DPOP Clinics
  • At each screening a chart is opened for each
    client
  • Anthropometrics and Lipid profile are collected
  • Each client is given
  • education regarding the results of labs collected
  • referral for medical follow-up for abnormal lab
    values
  • invitation to attend DPOP classes if found to be
    at risk or pre-diabetic
  • Clients encouraged to return and volunteer

  • Allow for make-up sessions

DPOP Future Development
  • Prospective follow-up of clients 3, 6, 9, 12
    months post completion of program.
  • Transfer operation of DPOP clinic site to
    community partner while maintaining QI by OCCHD
  • Meet educational needs of other races and
    cultures.
  • Seek needed resources to expand into other races
    and cultures
