  • To Schedule Transportation call

  • For Return Trips call 1-877-MHS-4U4U
  • After Hours/ Urgent Care Trips May Call
    1-800-508-7230 ext.6 or 1-888-732-2097
  • All Non-Emergency Transportation Services Must be
    Scheduled between 800a.m. and 500p.m. Monday
    thru Friday
  • Trips May be Scheduled out 30 Days in Advance

  • All Trips Non-Emergency Ambulatory and
    Non-Emergency Non-Ambulatory (Wheelchair) Must be
    scheduled by 400p.m. Two (2) Business Days in
    Advance
  • Also Trips over 50 miles Must be scheduled by
    400p.m. Two (2) Business Days in Advance

  • Urgent Care Trips can be provided Same Day if the
    PMP confirms that the Trip is Urgent in nature
  • Urgent Care Trips have a Two Hour Window for
    Pick-up from the PMPs Confirmation Time
  • After 3 Scheduled Urgent Cares, the member will
    need to re-schedule future Scheduled Urgent
    Care appointments
  • Normal Urgent Care request do not have any trip
    limits

  • Minor Children ages 0 thru 17 Must be accompanied
    by a 21 year old Parent or Guardian Escort or
    Authorized Representative
  • Car Seats are Required by Law for Children
    Newborn to Eight Years of Age
  • LCP Provides Booster Seats for Children 4 Years
    of Age and 40 Pounds to Eight Years of Age
  • Prior Approval is Required for Elective
    Procedures such as
  • Bariatric
  • Out-of-State Trips

  • LCP only Transports MHS Members to Medicaid
    Covered or MHS Authorized Services
  • Trips to the pharmacy are allowed Only when
    leaving the providers office for prescription
    pick-ups
  • Trips to non-Medicaid providers are covered for
    Methadone Treatment Only
  • MHS Members being Referred Must have the
    Facilities Name, Address, and Phone Number in
    order to Schedule the Trip

  • When Leaving a Message for LCP, Please Include an
    Area Code with the Phone Number
  • Transportation Vendor Claims Should be Sent to
    4308 Guion Rd. Suite D., Indianapolis, IN. 46254
  • Ambulance Claims should be submitted to P.O. Box
    531097, Indianapolis, IN. 46253-1097
  • Ambulance Inquiries should be faxed to
    317-291-6003.
