PRE-ADMINISTRATION Pre-ID Labels
IMPORTANT
  • Pre-ID information is provided to CTB by GaDOE.
If the Pre-ID Label is incorrect
  • Examiners must notify the School Test Coordinator
  • School Test Coordinator must update the school
    student records (AS400)
  • Pre-ID labels should match student being tested
EXACTLY. Be aware of similar names.
  • Do not use the label if information is incorrect.
    Barcode information will override bubbled
    information.
  • Barcode label may be used if the only inaccurate
information is the grade or teacher name.
  • If a student does not have a Pre-ID label,
information must be bubbled on the test book (Gr.
    1 2) or answer document (Gr. 3 8)
  • When bubbling demographic information, fill in
the text boxes first, then bubble.

19
PRE-ADMINISTRATION Pre-ID Labels

  • Barcode applied incorrectly
  • If answers have been gridded (i.e., student has
    bubbled in answers)
  • Transcribe student answers demographics onto a
replacement test book (Gr. 1 2) or answer sheet
    (Gr. 3 8).
  • Write VOID over the original booklet/answer
    sheet,
  • and return it with non-scorable materials.

sheet, and return it with non-scorable materials.

  • Small Group Kits consist of 10 test books of the

than the forms shipped to schools.
  • For Grades 1 2 the inclusion of the Examiners

    common form.

reported by Examiners to the School Test
    Coordinator.
  • The School Test Coordinator reports the
    irregularity to the School Principal.
  • The School Principal and School Test Coordinator
decide if the irregularity is severe enough to
report to the System Test Coordinator.
  • The School Test Coordinator completes the Testing
    Irregularities Documentation Form (provided
online at http//www.ctb.com/gacrct) and sends it
    to the System Test Coordinator.
  • The System Test Coordinator contacts the GaDOE
