  • Must serve individuals 60 and at least one
    target population of people with disabilities
  • Serve individuals at all income levels
  • Assist individuals planning for future LTC needs

  • Open a Resource Center meeting all the
    requirements in at least one community

Evidence-Based Prevention Program
  • 2003 AoA launched a new discretionary grants
    program
  • Grants will demonstrate the efficacy of
    delivering evidence-based prevention programs
    through community-based aging service providers.
  • Health promotion and disease prevention is a
    major AoA priority.

  • Public/private partnership with CDC, NIA, AHRQ,
    RWJ Foundation, Hartford Foundation, and the
    Horizon Foundation
  • 12 grants awarded in the following areas

  • All projects have succeeded in implementing
    interventions.
