  • IMAs records are maintained at Medical Facility
    supporting the unit of attachment

  • Send information to AFRC/SG

  • Provide the MTF with your orders transferring you
    to another base
  • Ask them to send your record to the new location

Readiness (PIMR)
  • Electronically entered in PIMR/DDSW/AFCITA
  • Physical exams (PIMR)
  • Dental exams (DDSW)
  • Medical readiness labs (PIMR)
  • Immunizations (AFCITA)
  • Improves the reporting of medical readiness rates
    to AD wing commanders
  • Enhances the visibility of IMAs for deployment
    and mobilization screening

Annual Requirements
  • Reserve Component Periodic Health Assessment
  • Mirrors AD PHA Program
  • Both Reserve Component Health Risk Assessment
    (RCHRA) and Dental Exam required
  • Requirements for all Participating Reservists
  • Military dental exam every 3 years
  • Civilian dental exam acceptable (using DD Form
    2813) on off years
  • Rated and Mobilization Assistants (MAs)
  • Specialized exams every year

Health Assessment
  • Reserve Component Health Risk Assessment
  • Four page risk assessment form (AF Form 4321)
  • Signed by you and physician, nurse, or technician
  • Standard Form 600 based on flying status
  • Dental exam (DD Form 2813)
  • Immunization record review
  • Bring these civilian test results, if available
  • Pap Smear
  • Mammogram
  • Optometry
  • Other test results

Physical ExamsYour Responsibility
  • Call DoD Medical Treatment Facility (MTF) to
    schedule exam
  • Schedule during Annual Tour, if possible
  • Follow preparatory instructions provided by MTF
  • 236 DoD MTFs available
  • http//www.tricare.mil/MTF
  • If youre told youre not eligible because you
    are not in DEERS
  • Youre still eligible for exams

  • Overprinted (RC)PHA SF 600
  • DD Form 2813 (Civilian Dental) or SF 603
  • Any extra exams or lab reports

  • Recommend you keep a personal copy

If You Fail to Comply
  • Your orders may be denied in AROWS by your
    Program Manager
  • Includes AT, IDTs, RPA and MPA tours
  • You could get unsatisfactory year for retirement
  • You could be recommended for reassignment to the
    Non-Participating IRR

Line of Duty Determinations
  • Verifies and determines your coverage, traveling
    directly to and from DUTY and during DUTY
  • IDTs
  • Annual tours
  • Special tours
  • School tours
  • MPA tours

Line of Duty Determination
  • If you become ill or injured
  • Inform your supervisor as soon as possible
  • Inform Program Manager
  • Report to the Military Treatment Facility
    immediately
  • Let the MTF know that you are a reservist
  • Get Line of Duty Determination (AF Form 348)
  • Also needed during a deployment
  • Once medical portion is complete forward AF 348
    to AFRC/RMG for further processing
