Cycle Deadline
  • Make sure that the application you have reflects
    the current cycle date and year. This
    information can be found on the cover of the
    application indicating the postmark deadline
  • Spring deadline date - March 24
  • Fall deadline date - September 11

Application Requirements
  • Read both pages of the requirements carefully.
    This section should help to answer most of your
    questions and help to explain the following
  • Award Amount
  • Selection Criteria
  • Eligibility
  • Required Documentation
  • Application Submission
  • Notification of Awards

  • Indicate the amount of money you are requesting
    (up to 19,000) on the top of the page. You
    cannot request more than you owe in educational
    debt.
  • Part A Fill out all of your personal
    information
  • this information will remain
    confidential.
  • It is important to note that you must be both a
    permanent California resident and a U.S. citizen
    to apply.

  • Part B List all relevant nursing and non-nursing
    work
  • experience in a medically
    underserved area.
  • Part C Provide information about your specific
  • background including family
    structure and
  • community where you grew up.
  • Part D Describe any language in which you are
    fluent.
  • This information needs to be
    verified by your
  • employer.
  • Part E You must type or write a personal
    statement
  • in response to the six questions
    listed. Your
  • entire statement should be no
    longer than
  • two pages.

Personal Statement
  • When writing your personal statement, consider
    these recommendations
  • Consider your goals as they relate to your chosen
    health profession.
  • Explain community outreach, documented volunteer
    service, and organizational memberships you have
    been involved with.
  • Tell about the communities and populations you
    currently serve or would like to serve and your
    cultural and linguistic experience in a health
    profession setting.
  • Explain the actual or potential difficulty you
    may have in completing your education/employment
    in the absence of an award.
  • Note Elaborate and provide as much
    information as you can. Personal
  • statements that lack
    detail may be considered incomplete and
  • therefore, ineligible.

  • Part F Complete the questions in this section.
    Note It is important
  • to note that your application
    will be ineligible if you indicate
  • that you currently owe an
    existing service obligation.
  • Service Obligation means the
    contractual obligation agreed to
  • by the recipient of
    scholarship or loan repayment where the recipient
  • agreed to practice
    their profession for specified period of time in
    or
  • through a designated
    facility.
  • Part G Read this section carefully and make sure
    to date and sign
  • below the certification
    statement. If this section is not
  • signed, the application will be
    deemed incomplete
  • Submission Checklist Use this checklist to
    ensure that all required documentation is
    included prior to mailing your application
    packet.

Employment Verification Form
  • This form should be completed by an
    Official in the Personnel or Human Resources
    Department. This form should be filled out in
    full before submission and include all of the
    following
  • Hour you worked per week in
  • direct patience care
  • Average weekly hours worked
  • Start date at this site
  • Fluency in a second language
  • Verification signature

Educational Debt Reporting Form
  • List all of the requested information which
    corresponds to your current educational loans.
    If your loans are consolidated, you only need to
    list the information for the consolidated loans.
  • You are also required to submit recent copies of
    the corresponding lender statements (dated within
    six months of the application deadline). Make
    sure your statements include your name, the name
    of the lender, balance owed, account number and
    monthly payments.
  • If the Educational Debt Reporting Form is not
    filled out completely or your lender statements
    do not include all of the required information,
    your application will be considered incomplete.

Incomplete or Ineligible Applications
  • Common mistakes which would deem an
    application incomplete or ineligible include the
    following
  • One or more of the required documents is
    postmarked beyond the current cycle deadline
  • Letters of Recommendation are not dated, signed,
    or formatted correctly.
  • Missing Official Transcripts
  • Application is not signed and/or dated
  • Student Aid Report or 2008 Federal Tax Return and
    all W-2s missing or not signed
  • Lender statements are unacceptable because they
    are not current or do not include all of the
    information specified in the application
  • Applicant does not follow the format or questions
    specified for the Personal Statement

Technical Assistance
  • The Foundation will convene a technical
    assistance call to discus the application process
    for this program. Please contact the Foundation
    staff to obtain the phone number and pass code if
    you would like to participate in this call.

    (FAQs) section of our website in order to find
    out more information about this program as well
    as the other programs offered by the Foundation.

Items to note
  • The Foundation encourages applicants to submit
    their application prior to the postmark deadline
    and recommends applicants contact the Foundation
    staff in order to verify that their application
    was received complete.
  • Applicants will be notified in writing within 120
    days of the postmark deadline whether or not they
    are selected to receive a scholarship award.
