JDC Population
  • Incarcerated youths arrested in Bexar Countymany
    from out of state
  • Age group 10-16
  • 192 beds population often gt200
  • Approximately 70 male, 30 female
  • Another 100 beds to be added in 2007
  • Crimes vary from runaway to murder but mainly
    violent offenses in JDC

  • Significant lack of support supervision
  • Many with distorted sense of right and
    wrongoften not taught
  • Many with poor social skillsoften not taught
  • High percentage with family members in jail or
    prison
  • Incarceration often thought to be acceptable or
    even a rite of passage

Upon admission to JDC
  • All youths screened for sexual activity and
    substance usage
  • Approximately 99 sexually active
  • Approximately 90 use alcohol or MJ
  • Often early onset of sexual activity (age 10-12)
  • Barrier method or any birth control rarely used
    by boys or girls

Urine CT GC testing
  • Urine collection is a mandatory part of admission
    procedure
  • Collection done on every admission, every time a
    youth is detained
  • Urine collection done at time of medical
    screening along with height, weight, vital signs
  • Youths advised we are not drug testing them

What happens if they refuse?
  • If they refuse, youth is sent to Security (a
    lockdown within a lock up)
  • Youths cannot go to General population until they
    cooperate with the complete admission procedure
  • Juvenile Detention staff supports us fully with
    this procedure

Urine CT GC testing cont.
  • Youths are told no news is good news
  • If () for CT and/or GC, youth brought to medical
    for tx and contact information
  • SAMHD faxed all ()s and contact info
  • If youth released from JDC, youths name and
    addressed faxed to SAMHD for f/u
  • High rate of recidivism in JDC

Juvenile Detention population
  • Many girls report boyfriend 10 years older
  • High percentage of coerced or forced sex
  • Often girls report being traded for favors or
    drugs
  • Many report 10-15 sex partners a month monogamy
    a rarity

Treatment for ()s
  • Patient teaching and risk reduction done at time
    of treatment
  • All ()s encouraged to also test for HIV, Hep B,
    and syphilis
  • All JDC youths are offered HIV testing while in
    our facility

Patient education and counseling
  • Weekly health classes taught by DHCS-Juvenile
    LVNs
  • STD education/prevention frequently taught along
    with contraception
  • Outside agencies encouraged to teach youths
  • High turnover of youths in JDC

  • All youths screened for sexual/physical abuse
  • Teen/high risk pregnancy often seen at JDC or CTK
  • Usually little to no prenatal care
  • Often girls have unrealistic expectations of
    pregnancy
  • Infants placed with family members or in CPS
    custody

  • Houses 14-16 year olds also (youths who are
    certified as adults)
  • 4294 bedsaverage daily population is 4200
  • 95 male, 5 female (women bailed out more often
    than men)

Significant Jail Health Problems
  • HIV()
  • Uncontrolled hypertension, diabetes
  • Cancer, renal failure, liver failure
  • Lack of any prenatal care, even in 3rd trimester
  • Significant health problems are often compounded
    by noncompliance of inmate
