Project Overview
  • California Community Choices is focused on
    developing Californias long-term care
    infrastructure to increase access to home and
    community-based services and to help divert
    persons with disabilities and older adults from
    unnecessary institutionalization

    Grant
  • Lead Agency California Health and Human
    Services Agency (CHHS)
  • Grant Period September 1, 2006- September 30,
    2011

Advisory Committees
  • A 15-member stakeholder Advisory Committee was
    established to advise CHHS on development of the
    project and to monitor project activities-
  • Expanded to 18 member January 31, 2008
  • A State Department Advisory Group includes
    representatives from the departments of Aging,
    Rehabilitation, Public Health, Health Care
    Services, Social Services, Developmental
    Disabilities and Mental Health, Housing and
    Transportation

Choices Mission
  • We are a statewide partnership committed to
    developing an infrastructure that will increase
    access to, capacity of, and funding for home and
    community-based services to provide all
    Californians with greater choice in how and where
    they receive long-term care services, in
    accordance with the Olmstead Principles.
  • The 1999 U.S. Supreme Court Olmstead Decision
    affirmed the right of individuals with
    disabilities to receive public benefits and
    services in the most integrated setting
    appropriate to their needs.

    recommendations for its long-term care system,
    featuring replicable and sustainable models that
    empower individuals through enhanced
    opportunities for choice and independence.

Goal 1 Improved Access to Long-Term Support
Services
  • Establish two resource centers, California Aging
    and Disability Resource Centers (CAL ADRCs) to
    provide a coordinated system of information,
    assistance and counseling for anyone seeking
    long-term services and supports (LTSS)

  • CAL ADRCs will perform core ADRC functions, but
    will have a more distinct focus on diversion
  • establishing a triage system among community
    partners that will link at risk individuals to
    community-based LTSS

  • Develop an integrated web portal (CalCareNet) to
    provide up-to-date information on LTSS
  • Will be piloted at the two CAL ADRCs
  • Commission a study to identify barriers to LTSS
    data integration at both the community and state
    levels and to identify what a model data storage
    warehouse would entail for California Data
    Warehouse Study

    information system which is fully accessible,
    comprehensive, easy-to-use, and consistently
