4SR Definition
- Assures that the proper condition of materiel in
storage is known and reported - Condition is properly recorded, and that the
materiel is properly provided with adequate
packaging protection to prevent any degradation
to lower condition codes - Process includes in-storage inspections, minor
repairs, testing, and packaging (COSIS) - Included are functions related to the receipt,
identification, classification, and packaging of
materiel during the receipt process - Stock Readiness excludes those actions that fall
under the area of general warehouse care and
depot maintenance, including the use of proper
storage aids, identification of materiel/storage
locations, and re-warehousing actions
5COSIS Definition
- Care of Supplies in Storage (COSIS)
- Ensures that materiel in storage is
maintained in a ready-for-issue condition or to
prevent uneconomic deterioration of unserviceable
materiel. With proper COSIS, supplies and
equipment in storage will be preserved and
maintained in a serviceable condition through
inspection and actions taken to correct any forms
of deterioration and to restore materiel to
ready-for-use condition. - COSIS includes the in-storage inspection, minor
repair, testing, exercising, preservation, and
packing of materiel, and all intra-depot materiel
movement to perform those tasks.
6Shelf Life
- Shelf-life items will be accomplished as
prescribed by chapter 4 of DoD 4140.27-M,
Shelf-Life Management Manual
7Receipt Process
- Shelf-life items are inspected to ensure proper
and sufficient shelf-life in accordance with the
DOD manual - If discrepancy is noted, a packaging Supply
Discrepancy Report (SDR) (SF 364) is prepared,
and place in CC L awaiting the determination of
disposition by the Administrative Contracting
Office (ACO).

- Shelf-life items are inspected to ensure proper
and sufficient shelf-life in accordance with the
DOD manual - Non Shelf Life Items

9COSIS Process
- If discrepancy is noted, the DD will prepare a DD
Form 1225, Storage Quality Control Report, and
forward those forms to the appropriate ICP/DSC SR
coordinator - If directed by the ICP/DSC, the materiel will be
forwarded for testing at an approved testing
facility. - The DD will prepare a DD Form 1222 and forward to
the ICP - The ICP will forward to the test facility, who
will complete the form and return to the ICP - The ICP will provide disposition to the DD
