Lecture on Health Knowledge of Bone Joint
Lecturer
2
  • Osteoarthritis, named
  • rheumatism involving bone by Traditional
Chinese Medicine physician, is the health
killer to middle ages and elders.

3
  • What is osteoarthritis?
  • Osteoarthritis, also known as degenerative
    arthritis, hyperplasia osteoarthritis or aging
osteoarthritis, is a kind of degenerative change
due to softened, disintegrated or locally
denudated cartilage matrix of joint as well as
insecondary hyperosteogeny at the joint edge.

4
  • It is characterized by joint pain and
    dysfunction, including joint malformation.
    Generally, diseases, including spur,
    hyporosteogeny, bowleg, cervical spondylosis,
    rheumatism, rheumatoid arthritis, for example,
slipped disc caused by degenerative change all
fall into the category of osteoarthritis.

5
  • Status of Osteoarthritis
  • Osteoarthritis is neither so serious as
cancer and AIDS nor so recognized as heart
disease and diabetes, but it troubles people,
racks patients, and ranks 1st among chronic
    diseases which disables adults.

6
  • Status of Osteoarthritis
  • Approximate 100,000,000 Chinese, which
accounts for 10 per cent of the national
population now suffer from osteoarthritis. Being
common among middle ages, elders and specific
occupations, including athlete and teacher and on
the rise among general population, it becomes the
major chronic disease which disables us and
affects our health and living quality.

7
  • As a chronic disease, osteoarthritis requires a
long period of treatment. Due to joint
dysfunction, patients are restricted for movement
and shall be replaced with artificial joints in
the late stage, which impairs patients living
and work ability greatly and brings heavy
financial burden to patients and their families.
According to statistics, by 1999, approximately
16,000,000 Americans out of the overall
    population of 280,000,000 suffer from
    osteoarthritis, the annual outpatients reach
80,000,000 person trips and 600,000 surgeries are
performed. The annual average treatment expense
is up to USD2,000 3,000 per capita.

8
  • ?The probability of disability
    resulting from arthritis is high.
    Critical patients
  • lose capability for working and independent
    living.
  • ?Arthritis is named Undying Cancer by WHO
  • ?Arthritis restricts the movement of middle ages
and elders and tends to induce cardiovascular and
    cerebrovascular diseases and diabetes after
    sitting for a long while.

9
  • In November 1999, it is officially approved by
UN Secretary-General Kofi Annan that 2000-2010 is
defined as the Bone Joint Decade, subsequently,
the Bone Joint Decade Plan is started by WHO
and every 12th October is named the International
    Arthritis Day.

10
  • Causes
  • Man cant stand without bone that cant move
    without joint.
  • The typical structure of movable joint is
comprised of articular surface, cartilage, cavity
    and capsule.

Articular cartilage
Where articular cartilage is the most important
component.
11
  • 1. Normal Articular Cartilage

Articular cartilage is a layer of smooth and
elastic tissue with 2-7mm of thickness covering
on articular surface. Working as a buffer with
decent elasticity or a perfect cushion between
joints, it bears various high-loading pressures
and impacts in cycle in our life. Healthy
articular cartilage guarantees the free motion of
the joint as well as buffers and protects the
joint by absorbing vibration energy resulting
from motion.
12
  • 2. Abnormal Articular Cartilage

With age, the articular cartilage of our body is
worn, lost and degenerated inevitably. As being
used and consumed over time, much more articular
cartilage is lost, but the self restoring and
rebuilding capacity is declining. In this case, a
part of cartilage becomes softened, eroded,
thinner and even disarticulated to produce a
tough surface and hyperosteogeny at the edge. For
lack of protection from cartilage, pressure
between joints increases, pain, swelling,
stiffness, sound or friction is occurred to the
joint both in still or in motion, and we will
fail to move as free as we did before.
13
  • Osteoarthritis
  • Ached in the joint, rooted in the cartilage

14
  • Clinical Manifestation
  • Symptom
  • It is mainly demonstrated by red, swollen,
heat, aching and dysfunction at the joint in
question, which becomes sensitive to change of
temperature and humidity in the climate.
  • Early stage patient feels slight discomfort
    and annoyed.
  • Common patient feels stiff, unbearable ache,
and be tired for walking up and down stairs.
  • Critical patient the joint is deformed, and
the joint moving range is reduced.

15
  • Main Symptoms
  • 1. Ache, Swelling
  • Ache is generally occurred in motion
  • and be exacerbated when walking up
  • and down and being loaded. Patients
  • are always waken by ache at night and
  • kept awaken the night through. With
  • ache, the joint motion is accompanied
  • by sound or friction frequently.

16
  • 2. Restricted Motion
  • It occurs slowly.
  • In fact, a great number of middle ages and
elders feel stiff in the joint when getting up in
the morning, several minutes or longer time is
required for restoration. Such phenomenon is
    called morning stiffness.
  • Patient feels stiff and ache in the joint
when standing up after a long time of sitting,
and is only allowed to move after minutes of
massage. So patients may fail to stand up at once
    in emergency.

17
  • 3.Dysfunction
  • With the growth of osteoarthritis, the joint
will become deformed or disabled for motion, and
finally lose all functions. Joints in critical
conditions shall be replaced with artificial
    ones.

18
  • Are you annoyed for
  • feeling ache at waist and legs in doing exercise?
  • feeling difficult in walking up and down stairs?
  • failing to have a trip?
  • daring not to turn over in the bed?

Unfortunately, you may be in trouble with
osteoarthritis if the motion of your joint is
always accompanied by ache, dryness,
inflexibility or even swelling and stabbing
19
  • Location
  • Osteoarthritis may be occurred to joints at any
part of the body from head to foot. Typically, it
is concentrated in cervical spine, hand, lumbar
    spine, knee and ankle joints.

20
  • X-ray film
  • 1. narrow gap between joints
  • 2. hyperosteogeny at the edge, i.e. spur
  • 3. hardened lower cartilage and degenerated
    capsule.
  • Clinical symptoms of osteoarthritis are not
necessary consistent with those illustrated by
the X-ray film. In practice, approximate 50 per
cent patients who suffering from osteoarthritis
    carries no distinctive symptom.

21
  • Pathogenesis
  • 1. The cartilage is degenerated and tenuated
with the age, but the self restoring capacity is
decreased. The incidence of osteoarthritis grows
as human and society become aged since aging
takes the most important contribution for
    osteoarthritis.
  • 2. The joint fails to bear pressures from
the body, and bones conflict with each other
without buffer. Obese people due to excess
nutrients and unscientific dietary habits in
modern society are much likely to suffer from
    Osteoarthritis.
  • 3. The incidence rate is high among special
    occupants, including athlete, teacher, dancer,
miner, female textile worker because joints are
    over used and damaged frequently.
  • 4. Osteoarthritis caused by infection, low
    immunity or wound, etc.

22
  • a.
  • It is found by medical research that
glucosamine is the basis of articular cartilage.
It is the raw material for composing and
producing cartilage and fluid and stimulates the
generation of cartilage cell for new cartilage.
  • The cartilage starts to aging since 25. With
the growth of our age, glucosamine is reduced
from the body, which may directly result in
    metabolic disturbance of cartilage cells
    meanwhile decomposition is faster than
    composition, the rebuilding and repairing
capability is decreasing, cartilage cells are
damaged or destroyed, and cartilage is worn,
degenerated and lost over time so that leads to
the degenerative change of articular cartilage
    and symptoms of osteoarthritis.

23
  • b.
  • Loss of bone calcium is one of the key factors
    for osteoarthritis
  • Healthy skeleton is featured with tight bone
quality and rigid bones. While the bone density
decreases, bone quality looks like alveolar and a
lot of calcium is lost for the incidence of
osteoporosis. Osteoporosis tends to cause ache at
waist, back and limbs and deformed antisternum
and always gives rise to fracture, disability so
that induces or exacerbates original chronic
diseases among elders. With very high lethality,
complication as the result of fracture is one of
main factors for the death of aged fracture
    patients.
  • Instead of calcium renewal, the key for treatment
is to inhibit the loss of bone matrix and improve
the physical conditions for calcium absorption
otherwise osteoporosis will be inevitable no
matter how much calcium is taken due to the
calcium absorbed from the external source is far
below than that to be lost.

24
  • c.
  • Dysfunction of blood circulation at local joint
  • For the inflammation of joint, local blood
circulation may be damaged severely and local
joint tissue may fail to get sufficient nutrients
so that results in lower self repairing capacity.

25
Osteoarthritis undying cancer The most
important cause for decreased living quality of
elders 9 out of 10 elders are osteoarthritis
patients
  • 360,000,000 people now surfers from arthritis
    all over the world.
  • Since 40 for female and 50 for male,
  • 55-60 is the highest incidence stage.
  • The incidence rate among elders over 70
    approaches 90 per cent,
  • which is far outweigh that of cardiovascular
    and cerebrovascular diseases
  • Furthermore, the number of patients is
    increasing

26
  • What shall be pointed out is that the
incidence rate of osteoarthritis is much higher
among female than among male, especially among
female at pre- and post-pausimenia stage.
According to statistics, women aged 45 is much
likely to suffer from osteoarthritis therefore
preventive measures shall be taken at about 35.

27
  • Status of treatment for bone joint diseases
  • The 1st Generation analgesia and
    anti-inflammation, symptomatic treatment
  • 1. Physical treatment simple treatment methods,
    including traction, massage, acupuncture,
infrared radiation and iontophoresis are applied
however these methods can only alleviate the
symptoms for a short while but fail to heal the
    articular cartilage.

28
  • 2. Medicine treatment
  • a. Oral medicine traditionally, non-steroidal
anti-inflammatory and antalgesic drugs are used.
  • It relives pain by inhibiting the process in
which pain is generated. Although it demonstrates
some advantages such as rapid pain killing
effect, such method can only maintain a short
while after stopping taking the drug because it
only inhabits the symptoms of pain and fails to
protect the articular cartilage. Some drugs would
even accelerate the growth of osteoarthritis.

29
  • For long-term use, some anti-inflammatory
drugs such as indomethacin may cause serious side
effects including nausea, vomiting, even gastric
    ulcer, gastrorrhagia and gastric perforation.
Such drug may damage liver and kidney function,
for example, causing acute renal failure and
imparie the hematopoietic function of the body
which results in dizziness, headache and
neurological disorder or even causes necrosis of
    the femoral head.

30
  • b. External medicine
  • Clinically, a lot of external medicines
including white and black salve mulls are used to
treat arthritis. Commonly used Longhu Bone
Strengthening Salve Mull and Musk Bone
Strengthening Salve Mull etc. relive pain and
improve local blood circulation effectively, but
they tend to damage the skin and fail to repair
cartilage fundamentally. Therefore, they cant be
    named ideal treatment methods.

31
  • 3. Surgical therapy
  • It is applicable to patients who have
serious deformed joints or need to be replaced
with artificial joints. Surgery brings patients
with great pain and financial burden.
  • Currently, it charges RMB 40,000-50,000 for
replacing single side of knee joint and RMB
80,000-100,000 for both sides. This brings great
pain and financial burden to patients and their
    families.

32
  • The 2nd Generation single ingredient,
    etiological treatment
  • Glucosamine renewa
  • Now, there are various glucosamine in
markets. Most patients will change the brand of
glucosamine after taking for a period of time if
no decent effect is received. In most cases, the
alternative one would fail to provide better
effect. This is because that most oral
glucosamine are single glucosamine which fail to
    renew bone and calcium fully.

33
  • 2. Calcium renewal
  • I always thought it would be rheumatism and
cold when I was troubled with bad back and waist
as well as cramp in legs. Now I know it is
because of osteoporosis, I need to renew calcium
  • The result of calcium renewal is continued
osteoporosis and insufficient calcium, and still
feeling tired to go up stairs

34
Is there any therapy which soothes symptoms and
tackles root causes so that relieves us from
troubles brought by osteoarthritis and
osteoporosis?
35
  • It is the time to cure osteoarthritis
  • with etiological treatment instead of symptomatic
    treatment

36
The 3rd Generation renewal bone and calcium,
etiological treatment
- repairs articular cartilage and inhibits
osteoporosis!
37
Ideal effect comes from perfect ingredients and
proportion
1.Glucosamine sulfate 2.Calcium
phosphate 3.Kudzuvine root extraction
38
  • 1. Glucosamine (sulfate) repairs cartilage,
    lubricates joint
  • Sales of glucosamine rank first among health
keeping products in 7 consecutive years in the
    United States.
  • Being approved by US FDA and EC, much more
glucosamine is sold than calcium renewal
    products.
  • Glucosamine is the only function food of
which the popularization of curative effect is
allowed by the government in Japan.
  • It is widely acknowledged by middle ages and
elders to renew calcium in China, but few of them
    know glucosamine renewal.

39
Glucosamine is a kind of mono aminosugar in
organisms, especially in articular cartilage. It
is the essential ingredient to compose
proteoglycan in articular mesochondrium of the
body. It is capable of inhibiting the
generation of superoxide radical that damages
cells, preventing the cartilage cells from being
damaged by cortical hormone and certain
non-steroidal anti-inflammatory medicines and
decreasing the release of endotoxin factors which
impairs cells soothing the pain of bone
joint, improving joint function and preventing
against the growth course of osteoarthritis and
promoting the generation of and renewing joint
fluid, keeping lubricating the articular
cartilage layer, reducing friction effect and
enabling the flexibility of joint. Glucosamine
controls over not only the bone joint health of
the body but also the metabolic balance of
articular cartilage and synovium.
40
  • There are two kinds of glucosamine in current
    market, namely D-glucosamine hydrochloride and
    D-glucosamine sulphate.
  • For treatment effect against arthritis,
    D-glucosamine hydrochloride receives general
effect but produces considerable side effects,
    especially for patients with hyperpiesia,
cardiovascular and cerebrovascular as well as
    kidney diseases
  • while D-glucosamine sulphate receives better
effect for reaching the nidus directly due to
higher purity, smaller molecule and easier
abosorption. The sylvine instead of sodium salt
in D-glucosamine sulphate produces no side effect
on patients with hyperpiesia, cardiovascular and
    cerebrovascular diseases, etc.
  • Glucosamine is extracted from natural and organic
raw materials, i.e. halobios in addition,
glucosamine is a kind of intrinsic matter of the
body, therefore it has no side effect on the
    body.

41
  • 2. Calcium phosphate renewal of calcium and
    phosphorus, enhancing bone density
  • Calcium renewal is the basic measure for any
kind of osteoporosis. Calcium phosphate contains
30-40 of calcium, which is beneficial for
    intestinal absorption.
  • Renewal of calcium and phosphorus is
particularly suitable for the status of middle
ages and elders. Calcium phosphate renews calcium
and phosphorus, prevents calcium and phosphorus
against loss, and strengthens bone density.

42
  • 3. Kudzuvine root extraction spasmolysis and
    dredge collaterals, promoting absorption

1. It is the absorptive accelerant for bone
joint and facilitates the absorption of
glucosamine and calcium. 2. It improves the
microcirculation in bone joint and increases
the supply of nutrition for abnormal cartilage
tissue. 3. It soothes pain effectively, and
relieves or prevents arthredema. It has decent
effects on decreasing lipid and pressure,
preventing thrombus, improving blood supply for
cardiac muscle, protecting against hearing loss
and promoting anamnesis, all of these are common
un-healthy states for middle ages and elders.
Therefore, it is characterized with multiple
functions, including repairing, protection,
treatment and prevention.
43
Kudzuvine root extraction contains a variety of
isoflavones so that works as the estrogen. Female
middle ages and elders are much likely to suffer
from insufficient calcium and osteoporosis for
loss of bone calcium due to decreased or
insufficient estrogen.
44
Laolaishou Glucosamine Kudzu and Calcium
Capsule consolidates the joint health
for you!
45
  • 4-step Treatment with Glucosamine Kudzu and
    Calcium Capsule

46
  • Step One Smooth
  • Smoothes joint channel, dredges blood vessel,
relieves spasm and improves blood circulation to
ensure the smooth of blood stream and the
nutrition supply for joint so that increases the
support function of joint and relives pain.

47
  • Step Two Elimination
  • Eliminates enzyme, inflammation factor and
free radical, etc. which damages cartilage from
joint cavity, promotes metabolism of bone
    quality, inhibits damage against articular
cartilage caused by free radical, and meanwhile,
eliminates a large quantity of sodion, controls
retention of water and sodium effectively and
    relieves or prevents arthredema.

48
  • Step Three Renewal
  • Renews glucosamine for cartilage and calcium
for bone quality in time, promotes the absorption
of glucosamine and calcium in joint under the
effect of kudzuvine root to achieve etiological
treatment as well as renewal of bone and calcium
so that accelerates absorption. Joints are
strengthened and enabled to move freely for the
bone flexibility and stiffness is improved.

49
  • Step Four Maintenance
  • Balances the inner environment of joint
cavity, lubricates joint, inhibits great loss of
    glucosamine and calcium, prevents the
reoccurrence of pathological process and conducts
comprehensive treatment so that enables the bone
to be healthier and stronger and demonstrate
    vitality again.

50
  • Clinical Verification of Glucosamine Kudzu
    and Calcium Capsule
  • A. Significant effect on cartilage repairing
  • Test for restoration of articular gap by
    comparing with placebo
  • Test process
  • Keep 211 osteoarthritis patients taking
2.0g/day Glucosamine Kudzu and Calcium Capsule
for three years and measure the articular gap of
patients with x-ray (see the image right). It is
found that the articluar gap is reduced from
5.39mm to 5.18mm, while the articular gap of the
comparison group is increased from 5.23mm to
    5.35mm.
  • Conclusion the articular gap restores to
the normal width and morning stiffness and pain
    disappear gradually after taking 2.0g/day
Glucosamine Kudzuv and Calcium Capsule for three
    years.

51
  • B. Antiphlogosis and odgnolysis, symptom soothing
  • Pain relief test by comparing with
    non-steroidal anti-inflammatory medicine
    Ibuprofen
  • Test process
  • A random and dual-blind clinical study
lasting for 6 weeks is conducted against 200
patients suffering from osteoarthritis in the
knee. Six weeks later, comparison is made between
the group which takes Glucosamine Kudzu and
Calcium Capsule 2,000mg/day and the group which
    takes non-steroidal anti-inflammatory medicine
    1,200mg/day.
  • Conclusion the pain relief effect is
identical for both groups after taking respective
medicines for 4 weeks, but six weeks later, the
pain relief effect of Glucosamine Kudzu and
Calcium Capsule is obviously superior to that of
    Ibuprofen.

52
  • C. Increasing bone calcium, improving
    osteoporosis
  • Effect on thigh bone weight, calcium and
density of rat by comparing with placebo and
    calcium carbonate
  • Pharmacodynamic Research Report of Shanghai
