information
There are a number of required fields to be
entered. Fill in the fields with the most
relevent option related to the issue/bug you are
reporting.

bug/issue
In the comment field, describe your reported
issue in detail. The more the better. Try to
describe what you were doing, what did you do
right before a problem occurred and what the
resulting problem was.
