about care. DH Holistic Care provides hassle-free
care from hospital transfer to home to providing
24hr support. We work collaboratively with you
and your care teams, such as family or GP, to
ensure you are well supported with the right care
for your needs. We focus our care on your
recovery and lifestyle according to your goals,
preferences, and needs.

Accommodation Services
A Home For You
At DH Holistic Care, we assist clients with
short, medium, or long-term accommodation. In
addition to our accommodation service, we provide
24/7 nursing, personal, or community support for
participants living in Shared or Supported
Independent Living home (s).
Enjoy Some You-Time We offer flexible
accommodations service that supports independent
and shared living options for NDIS or privately
funded participants. We understand the importance
of a HOME therefore, we aim to source suitable
accommodation that maximizes independence, so you
have a wide range of choices that suit your
living circumstances. All our accommodations are
conveniently located in Perth Metropolitan. Each
accommodation has its features and designs,
including safety and privacy, to fit our clients
preferences and needs.

Respite Care
We have flexible respite arrangements tailored to
meet both the caregivers needs and the persons
needs being cared for. To help support clients
and their families, our respite care does not
have a minimum duration. We can accommodate you
within one of our respite homes or respite in
your own home anytime and anywhere, whether you
need assistance for a few hours or a couple of
weeks. Our professional and caring staff are
ready to provide 24 hour/7hrs respite care, or
drop-in basis care, you choose, and we provide
services.
n addition to our 24/7 availability, our staff
can assist with your daily needs during your
stays with us, such as community participation,
nursing services, meals, personal care,
medications, and transport to and from
appointments/hospital.

Community and Social Support
Our approach is Family-first and for us
maintaining a supportive environment involves
connectedness with your community, family, and
friends. This service is for anyone wanting to
break the social barriers and build social
interactions with people within their community.
Whether you are looking to attend appointments or
errands, participate in bowling activities, etc.
Our support workers are there as a companion to
ensure you are safe.
