Intrauterine insemination (IUI)
  • IUI is an artificial insemination.
  • In this technique, sperm is directly inserted
into a woman's uterus when she is ovulating.
  • IUI is recommended when
  1. The males semen sample is slightly below the
    normal range.
  1. Females with ovulation problems or mild
    endometriosis.
  1. Couples have unexplained infertility.

5
intracytoplasmic sperm injection (I.C.S.I)
  • ICSI is Assisted Reproductive Technology (ART).
  • In this technique, sperm is directly injected
into the egg using a micromanipulator.
  • ICSI is recommended in following cases
  1. Ejaculatory problems
  1. Problems in getting an erection
  1. Extremely low or zero sperm count
  1. Oddly shaped sperms
  1. Reduced motility

6
in-vitro fertilization (ivf)
  • IVF refers to the fertilization process that
occurs outside the human body, in a plastic dish
    in the embryology laboratory.
  • IVF is recommended for
  1. Patients with failed artificial insemination
    trials
  1. Female factor, such as lesions or absence of
    fallopian tube, endometriosis or ovulation
    problems
  1. Male factor infertility, in which sperm count,
motility (speed) or morphology (shape) does not
    allow normal conception
  1. Failure to conceive after several years of
    attempts

7
Follicular Monitoring
  • It is a process to diagnose the number of mature
    eggs that will ovulate
  • It is useful for assessing the size of the
follicle that supports the growing egg and for
determining the thickness of the uterine lining.
  • In women taking fertility medication, follicle
monitoring is essential to assess her response to
    treatment.

8
Donor Egg Cycle
  • It is a process by which a woman provides one or
several eggs for purposes of assisted reproduction
  • Donor Egg Cycle is recommended when
  1. Women have age-related infertility (decreased egg
    quality quantity)
  1. Past failed IVF cycles
  1. Experiencing premature ovarian failure
  1. Genetic illness

9
Embryo Transfer
  • Embryo transfer technique includes the transfer
of the developing embryo(s) to the uterus.
  • After 3-5 days of development, the best embryos
    are selected
  • Embryo Transfer Highlights
  1. The best embryos are selected for transfer, after
a few days of development in external culture
    media
  1. The number of embryos selected for transfer
influences the pregnancy rate and chances of a
    multiple pregnancy.

10
Laser Assisted Hatching
  • Assisted Hatching facilitates the embryo hatching
process by thinning outer layer of embryo using a
laser beam, which may increase the chances of
    pregnancy.
  • Assisted Hatching could be considered in
  1. Older women, as they tend to have eggs with a
    thicker Zona Pellucida
  1. Repeated IVF-trial failure
  1. Embryo with thick Zona Pellucida
  1. Weak embryo quality in which self-hatching is not
    expected

11
P.E.S.A / T.E.S.A
  • P.E.S.A. is a technique for retrieving sperm from
the epididymis. No anesthesia is required.
  • If sperm cannot be found in the epididymis
(PESA), TESA is used to extract sperm from
testicular tissue. It is done under local
    anesthesia in the OT .
  • It is often used in cases where the male partner
    has one of the following
  1. no sperms are found in semen sample and there are
sperms in testis (blocked tube in male)
  1. primary testicular problems with deficient sperm
    production

12
Testicular Biopsy
  • It is a process to diagnose male infertility
  • It is also conducted to find the cause of low
sperm count or other hormonal imbalances.
  • It is recommended
  1. post sperm analysis
  1. When a blockage in the testicles may be causing
fertility problems which can be spotted and
treated with the help of a biopsy
  1. to retrieve sperm for IVF or ICSI
