What does SPARC do?

• SPARC works through community organizations and
  healthcare providers to promote disease
  prevention by
• Identifying people most in need
• Coordinating existing health services
• Developing new programs to increase access

How does SPARC do this?

• SPARC promotes basic disease prevention tools
  such as
• Vaccinations
• Cancer screening tests
• Cardiovascular tests
• Diabetes screening

SPARC is SUCCESSFUL!!!

• Previous areas with SPARC implementations has
  resulted in
• Increased delivery of pneumonia vaccinations by
  94
• Doubled the use of breast cancer screening among
  women attending flu clinics where SPARC offered
  mammography appointments
• Doubled the countywide rate of pneumonia
  vaccinations in a single flu shot season

Evaluation of SPARC

• SPARC aims to evaluate the reach, effectiveness,
  implementation, and adoption of the preventive
  services offered in each county.

  successful SPARC was and provide information for
  improvement of the programs.

• Services received at SPARC clinic
• Services received in the past (before SPARC)
