    centers (N 661 cohort-only patients)

  • 14 health centers (N 1710 cohort-only
    patients)
  • HDC 4 health centers (n 1059)
  • Non-HDC 10 health centers (n 651)
  • 97.8 type 2 diabetes
  • Ages 18 to 97 years mean age 60.50 years
  • 58.6 female
