• Builds on the foundation of LAP.
• Framework approach, collaborative working in true
  partnership model.

Multi-disciplinary Teams

• Built on an existing pilot in Exeter.
• Structure of organisation.
• Co-location.
• Perceived and actual benefits.

MDT moving forward

• True integration
• Clusters based around PBC.
• Integrated senior management level including a
  new post of Director of Health and Social Care.

Timescales?

• The project has three phases
• Initiation phase - the first being the
  development of the implementation requirements
  and plan (Jan 07 - April 07)
• Implementation phase the formal roll out of the
  workstreams in three stages based around locality
  hubs (May 07 to March 09) and
• Project closure phase the formal close and
  lessons learned of the project (March 09 May
  09).
