thereby offering their sponsors and donors tax
    receipts.
  • This can be a great incentives for both
    corporate and private donors.

5
How do we apply?
  • Complete the simple application form and submit
    to the foundation for review.
  • Information required by the Foundation outlines
your budget, proposed need for tax receipts, and
the proposed project beneficiaries (hockey teams,
    charities, CF, etc.)

6
Then what?
  • The Foundation Board will review the application
and approval decision will be made. (A foundation
member may contact your project chair or
secretary for more information if necessary.)
  • Approval is given in writing and temporary
receipt book(s) are sent out by the Foundation
    for clubs use.

7
How does it work?
  • When the project is completed, a financial
statement along with cheques and bank statements
need to be forwarded to the Foundation. Your
report should also include man-hours dedicated to
    the project (including committee meetings).
Finally, send a cheque for the total net profit
amount, made out to "District Four Foundation"
