What Happened? On the 29th of June 2004, an
employee suffered Severe fingers injuries when he
placed his hand on unguarded fan belts of a crane
engine that was suddenly started by his crew
member, this resulted in (1/3 amputation on the
middle finger and severe injuries on ring index
fingers).
Your Actions 1. Check and verify that
machinery with moving parts in your area
are fitted with safety guards. 2. DO NOT
operate any machinery with moving parts if
it is not fitted with proper safety
guards. 3. Any machine with broken/defective
guards must be removed immediately
from service until repaired. 4. When doing
your STOP Observation, lookout for any
unguarded machines.
