How Will It Work?
  • Obtain a Pay.gov User ID
  • Complete the form electronically on the Pay.gov
    Web site
  • Print out the completed form. Sign it. Date it.
  • Mail in the signed form along with any supporting
    documentation (such as Product Specification
    Sheets)
