Le Bonheur Childrens Medical Center

• 225 bed, acute care childrens hospital located
  in Memphis, Tennessee
• Founded June 15, 1952
• Part of the Methodist Le Bonheur Healthcare
  System
• Serves 95 counties throughout six states
• The hospital treats over 100,000 inpatient and
  outpatient children per year.

• Restructured community outreach programs to
  increase collaboration both internally and
  externally.
• Aligned with Government Relations to increase
  funding opportunities.
• Programs receive approximately 13 million in
  federal, state, and local funding on an annual
  basis.

• Funded by HRSA and TN Department of Health
• Goal Reduce morbidity and mortality associated
  with heart disease, cancer, diabetes, and stroke

• Started with seven counties in rural, West
  Tennessee
• Completed comprehensive needs assessment
• Identified highest risk groups
• Risk factors obesity, lack of physical activity,
  poor nutrition, lack of resources for
  prescription medications, lack of education, and
  unwillingness to make lifestyle changes

Fit for Life Network

• A formal, collaborative network of multiple
  health care and social service agencies located
  in rural, West Tennessee.
• Partners UT Extension Service Offices, Stanton
  Health Clinic, Lake County Primary Care Center,
  local school systems, and local health departments

Promising Results

• FYI - Over 900 students participated over 95
  increased knowledge on nutrition and fitness
• Dining with Diabetes - over 60 reduced
  cholesterol over 70 decreased A1c, blood
  pressure, and weight

Changes for 2005-06

• Addition of two counties and expansion of
  services with 1.6 million HCAP funding
• More structure to Network
• Chronic Disease Management and Partnership with
  Project Dulce

• Baptist Healthcare using FYI in four counties
• Participation in programs is growing - in McNairy
  County, Lace Up has doubled
• Addition of work site wellness
