  • The aim of this health check is to assist with
    the prevention of chronic disease and to enable
    early intervention strategies to be put in place
    where appropriate.

Item 717
  • Rebate 100
  • Claimed once only for each eligible pt
  • Claimed by a GP, but not for patients in hospital
  • Bulk billing incentive can be claimed for
    Commonwealth concession card holders
  • Cannot be claimed in conjunction with another GP
    attendance item on the same day, except where
    clinically required
  • Should be undertaken by usual doctor
  • PNs, AHWs and other health professionals may
    assist

Who is eligible?
  • Aged between 45-49 (inclusive)
  • At risk of developing a chronic disease
  • Specific risk factor must be identified, may be
  • Lifestyle risk factor
  • Biomedical risk factor high cholesterol, blood
    pressure, weight
  • Family history

What is involved?
  • Information collection pt history, examinations
    investigations
  • Overall assessment of the patients health
  • Interventions as indicated referrals and
    follow-ups of any identified problems
  • Provision of advice and information to the patient

  • Patient medical history
  • Family history
  • Lifestyle risk factors
  • Medication usage
  • Immunisation status
  • Psychosocial (depression)
  • Sexual and reproductive health
  • Family relationships
  • Work, environment and living conditions
  • Other history as necessary

Examination
  • Weight, height, BMI, waist circumference
  • Blood pressure
  • Skin check
  • Urinalysis
  • Sexual and reproductive health

Tests
  • Fasting lipids
  • Fasting blood glucose
  • Pap smear
  • Testing for colorectal cancer and breast cancer
    for at risk patients
