Risk Reduction/ Reap Rewards Program
  • Quarterly (or 2 times during year) follow up and
    re-screen with high risk people in the following
    Blood Pressure, Blood Cholesterol, Blood Glucose,
    Weight/Body Comp, (Optional Smoking).
  • SS sets goal with participant and coaches and
    gives education to help them meet the goal.
    Participant is rewarded once goal is met.

Living WELL Incentive Program
  • (Working Out, Eating Healthy, Living Long, Loving
    Life)
  • Wellness journals with a point incentive program
    will be given out to employees for behavior
    change and to help keep good habits. This program
    includes all employees. Journals will have a tear
    off page on back that can be turned in to
    employer and sent to SS Wellness for rewards.
  • This year long incentives program encourages
    continued good habits and moves high risk
    participants to change behavior and lower risk.
  • We will also include a monthly Wellness
    Newsletter for each employer group that can be
    sent to all employees.
