V. COMPLETING THE CONTRACT COVER SHEET

• Vendor information must be complete
• All required initials and signatures must be
  present
• All financial information must be complete

VI. COMPLETING A STANDARD AGREEMENT

• Fill in all necessary information where indicated
• Appropriate signatures must be present
• Description of services must be clear and concise
• Attach Recommendation for Award
• Must be submitted 10 days prior to start date

VII. NON-STANDARD AGREEMENT

• Must be submitted 15 days prior to start date
• Must not be signed by the department prior to
  legal review
• Must accompany a contract cover sheet
• Must attach the Alternative Dispute Resolution
  Clause

  amended
