Health Source RI Agency Coordinator
Completes Monthly Activity Report that includes
data from all sites in the agency, and transmits
to the Board of Elections. Responsible for
training of agency employees.
Site Coordinator (YOU)
Collects certification forms and registration
forms from site employees. Sends registration
forms directly to Board of Elections. Completes
Monthly Activity report and sends to the Agency
Coordinator.

packing slip to the BOE within 10 days of
completion by your client. - This become 5 days if the registration form was
signed within 5 days of the deadline to register
to vote. - Registration forms can be hand-delivered.

13Summary of Site Coordinator Responsibilities
- Coordinate the program at your office
- Monitor number of forms available
- Manage storage of certification forms
- Send a monthly report to the Health Source RI
Agency Coordinator - Send completed voter registration forms to the
Board of Elections - Notify us if another employee will be
coordinating the program.
14Helpful Tips for the Agency/Site Coordinator
- Plan on sending completed voter registration
forms to the BOE at least once per week. - If you will be out of the office, make sure a
co-worker completes the requirements outlined
today.
