Report The purpose of this document is to assist
schools in matching the demographics on Phase 2
hand-coded answer documents with the coding on
Phase 1 hand-coded documents.
The Phase 1 Hand-coded Demographics Report is
available April 721, 2010, via the Test
Administrator System on LEAPweb.
2Important information
The Phase 1 Hand-coded Demographics Report is a
report of the exact information coded on a
students Phase 1 answer document. In order to
match the Phase 1 answer document with the Phase
2 answer document, hand-coded student information
in fields E, I, and J must match exactly. If the
procedures are followed exactly and there are no
other extenuating circumstances, the student will
have one whole score, not a partial score. If a
student tested on a preidentifed answer document,
the student will not appear on the Phase 1
Hand-coded Demographics Report. If field E, I, or
J was not hand coded on a students Phase 1
answer document (all circles for the field are
blank), the student will not appear on the Phase
1 Hand-coded Demographics Report. You must
hand-code demographic information in fields E, I
and J to appear on this report. Do not leave any
demographics blank.

- TIMING
- The Phase 1 Hand-coded Demographics Report is
available to districts and schools via LEAPweb
April 721. - LEAP/GEE Phase 2 materials will arrive in
districts March 23-24. It is strongly advised
that the hand coding of Phase 2 answer documents
be scheduled to take place after this report
becomes available. - Phase 2 testing begins April 12.
- Access to the report via LEAPweb will end on
April 21.
4Important information (CONTINUED)
- ALERT Even if the Phase 1 Hand-coded
Demographics Report shows errors in student
information, student information in fields E, I,
and J on Phase 2 answer documents must be hand
coded exactly as it appears on this report. (See
the example on slide 9 of this guide.) - If a non-preidentifed student does not appear on
the report, the District Test Coordinator should
contact LDE for further information. - District Test Coordinators and School Test
Coordinators should keep this record of errors in
student information as an alert of students whose
demographics will need to be corrected in Student
History.
5Phase 1 Hand-coded Demographics Report
Hand-coded student demographic information in
fields E, I, and J must match exactly on Phase 1
and Phase 2 answer documents for a student.
This report is provided via LEAPweb to assist
test coordinators and test administrators when
hand coding student demographic information on
page 1 of nonpreidentified Phase 2 answer
documents.

6How to access the report ...
- Sign onto the LEAPweb application.
- Click on the Test Administration tab.
- Then click on the link for the Hand-coded
Demographics Report on the left side of the
screen.
7District report (PDF file format)
- District Test Coordinators will have access to
Phase 1 hand-coded student information for all
schools within the district. The district-level
report is contained in a single file. - The district report is sorted so that information
for site codes 999 (Non Public), 998 (Home
Study), and 888 (Unknown) appears at the top of
the report. Reports for all other site codes in
the district follow in ascending order. - If a student moved from one school to another
within the district after the completion of Phase
1 testing, the District Test Coordinator will be
able to search the district file for the
students name and provide the Phase 1
demographic information to the School Test
Coordinator at the students new school. - If a student moved into the district from another
district after the completion of Phase 1
testing, the District Test Coordinator should
contact LDE to obtain the Phase 1 demographic
information for the student.
8School report (PDF file format)
- School Test Coordinators will have access to
Phase 1 hand-coded student information for their
school only. - If a student moved from one school to another
within the district after the completion of Phase
1 testing, the School Test Coordinator at the
students new school must contact the District
Test Coordinator, who will provide the Phase 1
demographic information. - If a student moved into the district from another
district after the completion of Phase 1 testing,
the School Test Coordinator must contact the
District Test Coordinator, who will contact LDE
to obtain the Phase 1 demographic information for
the student.
9Using the report ...
- Information in this report must be used to hand
code student information on the Phase 2 answer
document for each LEAP and GEE student whose
Phase 1 answer document was also hand coded. - ALERT Code fields E, I , and J on each students
Phase 2 answer document using information exactly
as shown in the shaded columns on this report.

you know the information that is missing. - Do NOT correct errors in student information as
you hand code documents using this report.

bar-code labels. Use the bar-code labels that
