Phase 1
  • No dedicated staff members
  • Delivered by CCU staff prior to transfer when
    time and staffing allows
  • Offer
  • BHF booklets, DVD, mini E.C.G. contact numbers

Phase 2
  • Email from Morriston x checked with WGH
    admissions
  • Telephone call 2-5 days after receipt
  • Home visit or clinic within 2 weeks by either
    nurse
  • R2R
  • Referral to PHCT

Phase 3
  • Between 1-4 week wait
  • 1free hospital based weekly session,
  • 2 community halls, 3 leisure centres 1
  • All offer education sessions
  • OT offers relaxation sessions 2-3 times/month
