What Linkages looks like today
  • Co-location of CalWORKs and Voluntary Child
    Welfare Services (CWS)
  • All voluntary CWS cases are screened for CalWORKs
    eligibility
  • Cases with mutual customers are identified as
    crossover cases and referred to an integrated
    team
  • Increased communication

  • Awareness of resources and strengths
  • Reduction of barriers and service duplication
  • Sharing of information, advocacy, and
    appreciation

  • Emergency triage team
  • Coordinated Case Plan (CCP) with
    Multi-Disciplinary Team within the first 30 days
  • Referrals to Behavioral Health (AOD, MH, DV) to
    establish appropriate funding streams
  • Continuum of care
