We are comprised of two components

• Area Health Education Center (AHEC)Links medical
  students and the medical school to the community
  through health education and non-medical
  community service
• Community Health Programs (CHP)Provides free or
  low-cost health services to residents of
  Southeastern Ohio.
• In 2005, CHP and AHEC united under one director,
  Kathy Trace, MHA.

CHP Staff

• Each of our programs is coordinated by our highly
  qualified and dedicated staff.
• We are continually developing new programs to
  better meet health needs in Appalachia.

Background

• In 1994, the Childhood Immunization Program began
  with one mobile medical unit.
• The program grew in subsequent years to meet
  other health needs in the area.
• In 2001, a second mobile medical unit was added.
• The efforts were renamed Community Health
  Programs in 2006.

  adolescents
• Assists families in determining eligibility for

Community Health Programs

• Educates and consults child care providers on
  health and safety issues
• Provides free hearing and vision screenings and
  medical referrals for children ages 3-6

Perinatal Initiative Overview
Community Health Programs

• Provides perinatal education to high-risk
  pregnant women at and below the 200 poverty
  level who are uninsured or underinsured
• Assists with smoking cessation, nutrition,
  exercise and prenatal education services
• Provides depression screenings, social and
  emotional support, case management and referrals
  to other community agencies
• Since the programs inception in 2000, preterm
  birth rates in Athens County have dropped below
  state and national level

Childhood Immunization Project (CHIP) Overview
Community Health Programs

• Provides free routine immuni-zations to children
  aged 0-18, regardless of insurance status
• Covers a 21 county-wide area of Southeastern
  Ohio
• Assists in kindergarten registration and referrals

ComCorps Overview
Community Health Programs

• Began in 1999 with a planning grant
• Partners with schools and community agencies to
  provide vision, lice and hearing screenings
  school and community health education and
  health-care access to Athens County children and
  families
• Serves as a domestic Peace Corps, providing
  individuals with a living stipend and education
  award in exchange for one year of service
• Received an Ohio portfolio peer review of
  100only state program to do so

• Conducted 19,047 health education encounters with
  Athens county residents in 2007
• Provided 15,214 individual screenings for lice,
  hearing, vision, gross motor skills, and
  immunization compliance
• Provided 3,960 individuals and families with
  access to healthcare opportunities
• 20 ComCorps members and 322 volunteers served a
  combined 25,202 hours estimated at 454,644

Breast and Cervical Cancer Screening Program
(BCCP) Overview
Community Health Programs

• Utilizes a mobile medical unit, as part of the
  Healthy Adult Program
• Offers screening and education for uninsured and
  underinsured women ages 40-65 (and others who
  qualify) for breast and cervical cancer
• Provides referrals and follow-ups
• Presents information about womens health issues
  at schools, community agencies, senior centers
  and businesses
• Collaborates with health care agencies and/or
  other community agencies

• Raised awareness through 31 health fairs,
  reaching a total of 2,321 people
• Spoke to 561 students at 13 high schools about
  breast and cervical cancer
• Screenings resulted in the detection and
