• To schedule transportation call 1-800-508-7230
• For return trips call 1-888-732-2097
• After Hours/ Urgent Care trips may call
  1-800-508-7230, ext. 6 or 1-888-732-2097, ext.1
• All non-emergency transportation services must be
  scheduled between 800 a.m. and 500 p.m. Monday
  thru Friday

• Trips may be scheduled out 30 days in advance
• All trips, non-emergency ambulatory and
  non-emergency non-ambulatory (wheelchair), must
  be scheduled by 400 p.m. two (2) business days
  in advance
• Trips over 50 miles
• must be scheduled by 400 p.m. two (2) business
  days in advance, and
• require prior authorization.

• Urgent Care trips can be scheduled same day if
  the PMP confirms that the trip is urgent in
  nature
• Urgent care trips have a two- hour window for
  pick-up from the PMPs confirmation time

• Minor children ages birth thru 17 must have
  parental or guardian escort or show proof of
  legal emancipation
• Car seats are required by law for children
  newborn to eight years of age
• LCP provides booster seats for children 4 years
  of age and 40 pounds to 8 years of age
• Prior approval is required for elective
  procedures such as
• Bariatric
• Out-of-State Trips

• LCP only transports Anthem members to Medicaid
  covered or Anthem authorized services
• Anthem members being referred must have the
  facilities name, address, and phone number in
  order to schedule the trip
