What is Keep Well?
  • 3 year programme funded by the Scottish
    Government
  • A model of anticipatory care in practice
  • Aims to increase the rate of health improvement
    in 45-64 year olds in areas of greatest need,
    reducing health inequalities
  • Particular focus on early intervention for those
    at high risk of CHD and stroke
  • Strong association between CHD and inequalities
  • with an increased risk for people from the most
    deprived areas in comparison to the risk for
    people living in the least deprived areas
  • Based on the Dr Julian Tudor Hart model to
    address the inverse care law
  • Higher disease burden in deprived areas not
    matched by proportionate use of services
  • .people who need most get least

    the most deprived 15 Scottish Index of
    Multiple Deprivation

  • 1 million per annum to deliver Keep Well
  • Keep well VisionTo increase the rate of health
    improvement in deprived communities by enhancing
    primary care services to deliver anticipatory
    care by -
  • identifying and targeting those at particular
    risk of preventable serious ill-health (including
    those with undetected chronic disease)
  • offering appropriate interventions and services
    and
  • providing monitoring and follow-up

  • Living in 20 most deprived datazone areas
  • Hard to reach population homeless, travelling

    between most affluent and most deprived
  • Increasing health inequality ratio despite
    general decrease in CHD mortality

  • Different to all other Keep Well sites (Waves I

  • Reflects Fife Council ROA
  • Target population 17,212 (including disease
    registers)
  • Hard to engage populations included
    travelling, homeless
  • Potential engagement with 50 GP Practices across
    Fife
