How to Access the On-line Site
The new on-line contribution form is accessible
from the Utah State Employees Charitable Fund
Site at http//www.usecf.state.ut.us/index.htm
beginning October 1, 2007.

Accessing the On-line Site
Once at this website, simply click on the link
Click Here to Enter your 2008 Campaign Pledge
Online.

Logging in
You will be prompted to enter your login. To
login, simply enter your login and network
password, just as you would to enter your
timesheet or access any other such state website.
NOTE Only those employees who were active in
payroll as of Sept 27, will be able to use this
on-line contribution form, as users are validated
against this payroll extract to ensure that
payroll deductions can be completed successfully.

Entering a Contribution
After you have logged in, you will see the
following screen. To contribute via Payroll
Deduction, simply click on the line Payroll
Deduction Contribution.

Entering a Contribution
This will take you to the Pledge and Payroll
Deduction Form. You will be prompted to enter a
Charitable Agency Code and an Annual Amount.

Entering a Charitable Agency Code
If you know the Charitable Agency Code, then
enter it, otherwise press the Search button.

Searching for a Charitable Agency Code
After pressing the search button, you will be
taken to the following page. In the search box
you can enter any key word and then press the
search button.

Choosing a Charitable Agency
All organizations with that keyword in the title
will be listed directly below. When you see the
Charitable Agency you wish to contribute to,
simply click on the agency code.

Entering your Contribution Amount
You will return to the pledge form and the
Charitable Agency Code entry box will contain the
value that you selected from the Search page.
Now, enter the Annual amount that you wish to
contribute in the Annual Amount entry box. Then
press the Add Entry button.

Entering your Contribution Amounts
Once you have pressed the add entry button, the
entry will display in the list box directly
below. The system will automatically divide your
annual amount and display the correct amount that
will be deducted per paycheck. Due to the fact
that each payroll deduction must be an equal
amount, the system may have to round and the
amount may be slightly different than you
entered. (Such as 199.94 for an annual amount,
when you entered 200.00)

Entering Contribution Amounts
The system will allow you to continue to add new
entries using this same process. It will also
allow you to either edit or delete your entries.

Editing an Entry
To edit an entry, simply press the word EDIT next
to the entry. The system will fill in the
Charitable Agency Code and Annual Amount from the
line that you chose to edit. You can overwrite
either the Charitable Agency Code or Annual
Amount or both. Once finished, just press the
Edit Entry button.

Deleting an Entry
To delete an entry, simply press the word DELETE
next to the entry. The system will fill in the
Charitable Agency Code and Annual Amount from the
line that you chose to delete. If you are sure
that you wish to delete, press the Delete Entry
button.

Submitting the Form
Once you are finished entering your data, press
the Submit Donation button, this will indicate
that you are ready to submit your entries for
payroll deduction. If you exit the application
BEFORE Submitting your donation, the donation
will not be finalized and you may return at
anytime during the Charitable drive to finalize
your donation.

Confirming your Donation
Once you have submitted your donation, the system
will show you a review screen. This screen lists
the entries that you have made and will allow you
to either go on and confirm, by pressing the
Confirm Donation button or go back and edit
entries by pressing the EDIT button.

Printing your Receipt
Once you have confirmed your donation, an on-line
receipt will display. To print the receipt,
simply press the PRINT button. When you have
printed your receipt, press the FINISHED button
to exit and return to the main page. Once you
have pressed the FINISHED button, you will not be
able to return to the receipt page, so make sure
you print before you leave this page.

After you Confirm your Donation
Once you have confirmed your donation, you will
notice that the option to enter contributions via
Payroll Deduction is no longer available. In
order to eliminate the chance of duplicate entry,
the option disappears once the contribution has
been confirmed. If you need to make changes to
your contribution once you have already
confirmed, contact your agency representative
listed on your receipt.

Entering One-Time Donations
To enter a one-time contribution, simply click on
the line One Time Contribution.

Entering One Time Contributions
From here, all options are the same as with the
payroll contribution screens. You may search,
add entries, delete, edit, submit and confirm
your contribution in the same way as outlined
under the instructions for payroll deduction.
There is one exception, the amount WILL NOT be
divided into equal payroll deduction amounts.
You will enter the EXACT AMOUNT that you wish to
contribute in a single donation.

Entering One Time Contributions
In addition, the instructions for submitting your
donation are different. If you contribute via
One Time Contribution, you will be asked to
attach a cash envelope or check to your receipt
and turn those in to your agency representative.
